The Compass Bioscience Hemoglobin A1c Control is to be used as a quality control material to assess the accuracy and precision of laboratory test methods used to measure Hemoglobin A1c levels. It is intended to validate the measurement of Hemoglobin A1c in patient samples. Two levels of control are provided to allow the performance of the analyte test methods to be monitored within the clinically significant range.
Device Story
Hemoglobin A1c Assayed Control consists of human whole blood matrix adjusted to target HbA1c concentrations; includes stabilizers and preservatives. Supplied as frozen liquid in two levels; ready-to-use; no reconstitution required. Used in clinical laboratories to monitor performance of HbA1c assay procedures; validates measurement accuracy and precision within clinically significant ranges. Healthcare providers use control results to verify instrument/reagent performance before reporting patient HbA1c values; ensures reliability of diagnostic testing for diabetes management.
Clinical Evidence
No clinical data. Bench testing only. Value assignment performed via HPLC reference method over 30 days across multiple instruments. Stability validated via accelerated (Kennon model) and real-time studies.
Technological Characteristics
Liquid human whole blood control; bi-level (normal/elevated); 3 mL per level. Storage: -15 to -25°C (2 years) or 2 to 8°C (12 months). Open vial stability: 90 days at 2-8°C or 7 days at room temperature.
Indications for Use
Indicated for use as a quality control material to assess accuracy and precision of laboratory test methods for measuring Hemoglobin A1c levels in patient samples. Intended for clinical laboratory use.
Regulatory Classification
Identification
A quality control material (assayed and unassayed) for clinical chemistry is a device intended for medical purposes for use in a test system to estimate test precision and to detect systematic analytical deviations that may arise from reagent or analytical instrument variation. A quality control material (assayed and unassayed) may be used for proficiency testing in interlaboratory surveys. This generic type of device includes controls (assayed and unassayed) for blood gases, electrolytes, enzymes, multianalytes (all kinds), single (specified) analytes, or urinalysis controls.

I. Device Description:
The Compass Bioscience Hemoglobin A1c controls are a bi-level (normal and elevated) control (3 ml per level) solution containing glycated hemoglobin. The controls are supplied as liquid human whole blood samples. All donor units used in the manufacture of this product have been tested using FDA accepted methods and have been found non-reactive for Hepatitis B surface antigen (HBsAg), antibodies to Hepatitis C, HIV 1/2 and syphilis, and for HIV 1 and HCV RNA.

{2}
3. Comparison with predicate:
The Compass Bioscience Hemoglobin A1c control is similar to the predicate devices in regards to intended use, matrix (liquid whole blood), levels (2), volumes (3 x 1 mL) and storage temperature (-20°C). The Compass Bioscience is different in that the open vial stability is 90 days when stored at 2-8°C and 7 days when stored at room temperature. In contrast the MAS predicate device has a 14 day open stability when stored tightly capped at 2-8°C.

c. Traceability, Stability, Expected values (controls, calibrators, or methods):
The Compass Hemoglobin A1c Controls are prepared from human whole blood from traceable normal and diabetic donors. The concentrations of HbA1c are determined using HPLC as the reference method.
Value Assignment
The value assignment was determined by testing the controls over 30 days, for both refrigerated and room temperature stored controls, at least 10 data points for each level. The means were calculated and the ranges determined by +/- 3SD of the mean. Testing on the Beckman instruments, the Bayer DCA 2000, the Hitachi 912, the Tosoh 2.2, and the Variant II were performed at individual outside labs on a single instrument. The testing on the GDx and the Tosoh G7 was performed in-house, and the testing on the In2it was performed by the instrument manufacturer.

Accelerated and real-time stability studies have been performed to determine the stability and shelf life for the controls. Frozen control samples were allowed to thaw at room temperature (20 to 25°C) before testing. The accelerated study was conducted using the Kennon stability prediction model. The results from the accelerated closed vial study support the sponsor's claim of 2 years when stored at -15 to -25°C and 12 months when stored at 2 to 8°C. The results from the real-time open vial stability study support the sponsor's open vial claim of 90 day stability when stored at 2 to 8°C and 7 day stability when stored at room temperature.
