Intended Use

The Paladin System with Integrated Embolic Protection (IEP) is indicated for Percutaneous Transluminal Angioplasty (PTA) in the carotid arteries with capture and removal of embolic material. This device is also indicated for post-dilation of self-expanding stents in the carotid arteries with capture and removal of embolic material. The diameter of the arterial site for filter deployment should be no more than 7.0 mm. The Paladin System with IEP should always be used in conjunction with an available embolic protection device.

Device Story

Paladin System is a sterile, single-use, rapid exchange (RX) PTA catheter; features nitinol-based embolic protection filter located between PTA balloon and distal tip. Physician operates device under fluoroscopic guidance; catheter advanced over guidewire to carotid stenosis. Handle at proximal end controls filter activation. Balloon inflates to specific diameter/length to dilate artery or post-dilate self-expanding stents. Integrated filter captures embolic material during procedure; removed with catheter. Used in conjunction with a primary distal embolic protection device. Benefits include simultaneous angioplasty/stent post-dilation and embolic protection, reducing procedural steps and potential for embolic events.

Clinical Evidence

Multi-center study (n=106) in Germany evaluated safety/performance in adults with carotid artery stenosis. Primary endpoints: 30-day major adverse event (MAE) rate (stroke, MI, death). Results: 30-day MAE rate 0.98% (one event unrelated to device); procedural success 100%; technical success 99%. Bench testing included dimensional verification, balloon fatigue, burst pressure, embolic capture efficiency, and stent compatibility. Animal studies (swine model, n=13) confirmed histological safety at 30 days and 6 months.
