Intended Use
The ENDOSKELETON® TA Interbody Fusion Device is indicated for use in skeletally mature patients with Degenerative Disc Disease (DDD) at one or two contiguous levels from L2-S1. Patients should have received 6 weeks of non-operative treatment prior to treatment with the device. The Endoskeleton® TC is indicated to be used with supplemental fixation and autograft bone. The Endoskeleton® TL IBD is indicated for use in spinal fusion procedures in skeletally mature patients with Degenerative Disc Disease (DDD) at one or two contiguous levels from L2-S1. DDD is defined as discogenic back pain with degeneration of the disc confirmed by patient history and radiographic studies. Patients should have received 6 months of non-operative treatment prior to treatment with the device is intended for use with supplemental fixation systems cleared for use in the lumbar spine. Device Story
Endoskeleton System comprises interbody fusion devices and vertebral body replacements; used in spinal fusion or reconstruction procedures. Implants feature microscopic roughened surface with nano-scale features; manufactured from Ti6Al4V-ELI titanium alloy. Devices implanted by surgeons in clinical settings to stabilize spinal segments or replace damaged vertebral bodies. Provides structural support for bone fusion; used with autograft or bone graft substitutes. Supplemental fixation required for most models; TAS model functions as standalone system with integrated screws. Benefits include stabilization of spinal segments, pain reduction, and restoration of vertebral height.
Clinical Evidence
Bench testing only. Mechanical strength validated per ASTM F2077 (axial fatigue). Biocompatibility established per ISO 10993-1 and animal implantation model. Wear testing performed using modified ASTM F1160 protocol with particulate characterization.
Technological Characteristics
Materials: Medical grade titanium alloy (Ti6Al4V-ELI). Surface: Microscopic roughened with nano-scale features. Standards: ASTM F2077 (mechanical), ASTM F1160 (wear), ISO 10993-1 (biocompatibility). Form factor: Various sizes/geometries for spinal anatomy. Energy source: None (passive implant).
Indications for Use
Indicated for skeletally mature patients with Degenerative Disc Disease (DDD) at one or two contiguous levels (L2-S1) or cervical spine (C3-C7), or vertebral body replacement (T1-L5) for tumor/trauma. DDD patients must have discogenic pain, radiographic degeneration, and failed 6 months (lumbar) or 6 weeks (cervical) non-operative treatment. May include Grade I spondylolisthesis/retrolisthesis. Contraindications include skeletal immaturity.

An intervertebral body fusion device is an implanted single or multiple component spinal device made from a variety of materials, including titanium and polymers. The device is inserted into the intervertebral body space of the cervical or lumbosacral spine, and is intended for intervertebral body fusion.
Special Controls
*Classification.* (1) Class II (special controls) for intervertebral body fusion devices that contain bone grafting material. The special control is the FDA guidance document entitled “Class II Special Controls Guidance Document: Intervertebral Body Fusion Device.” See § 888.1(e) for the availability of this guidance document.(2) Class III (premarket approval) for intervertebral body fusion devices that include any therapeutic biologic (e.g., bone morphogenic protein). Intervertebral body fusion devices that contain any therapeutic biologic require premarket approval.

This traditional 510(k) is intended to modify the surface treatment of Endoskeleton devices.
The Endoskeleton system is an interbody and vertebral body system comprised of a variety of implant sizes and geometries to accommodate various patient anatomy and pathology. The subject Endoskeleton® device is identical to the predicate devices with respect to indications for use, design, dimension, and materials. The only difference to the currently marketed devices is the change in surface treatment.
# Technological Characteristics
There are no changes between the predicate devices and the subject devices with respect to indications for use, design, dimension, and materials. The only difference to the currently marketed devices is the change in surface treatment. There are additional processing steps for the new surface treatment when compared to the previous surface treatment.
# Performance Testing:
Mechanical testing was performed to establish mechanical strength of the devices (ASTM F2077). Surface specific characterization and integrity testing were completed to support biocompatibility through ISO 10993-1. Pre-clinical testing performed included axial fatigue testing per ASTM F2077. Additionally, wear testing was done using a modified protocol of ASTM F1160 with particulate characterization. Biocompatibility per ISO 10993 was established through ISO 10993-1 assessment and an implantation animal model. The testing indicated that the Endoskeleton® devices are adequate for the intended use and substantially equivalent to the predicate systems.
