Indications for Use

Device Story

Sentinel Pancreatic Amylase is an in vitro diagnostic reagent kit for quantitative analysis of pancreatic amylase in human serum or plasma. Used on Abbott Aeroset and Abbott Architect c8000 systems. Employs two-step enzymatic colorimetric assay; incubation step uses monoclonal antibodies to inhibit salivary alpha-amylase; reaction step uses defined oligosaccharides (4, 6-ethylidene-G7pNP) cleaved by pancreatic alpha-amylase; resulting fragments hydrolyzed to p-nitrophenol (pNP) and glucose by alpha-glucosidase. Increase in absorbance proportional to pancreatic alpha-amylase activity. Results provided to clinicians to assist in diagnosis and treatment of pancreatitis.

Clinical Evidence

Bench testing only. Precision studies (intra-assay and inter-assay) performed on Aeroset and c8000 systems with CV < 6%. Linearity established up to 2200 IU/L. Method comparison studies (n=72 for Aeroset vs predicate, r=0.9994; n=56 for c8000 vs Aeroset, r=0.9998) demonstrate high correlation. Analytical specificity tested against hemoglobin, bilirubin, and triglycerides with no interference at specified concentrations.

Standard/Guidance Document Referenced (if applicable): None referenced ## L. Test Principle: This is an enzymatic, colorimetric two step assay. In the incubation step, the activity of the human salivary $\alpha$-amylase is inhibited by two different monoclonal antibodies without affecting the pancreatic $\alpha$-amylase. In the reaction step, defined oligosaccharides such as 4, 6-ethylidene (G7) p-nitrophenyl-pNP (G1) $\alpha$, D-maltonepatoside (ethylidine-$\mathrm{G}_7\mathrm{PNP}$) are cleaved under the catalytic action of $\alpha$-pancreatic amylases. The $\mathrm{G}_2\mathrm{PNP}$, $\mathrm{G}_3\mathrm{PNP}$ and $\mathrm{G}_4\mathrm{PNP}$ fragments that are formed are completely hydrolyzed to p-nitrophenol (pNP) and glucose by $\alpha$-glucosidase. The increase of absorbance due to pNP formation is proportional to the pancreatic $\alpha$-amylase activity in the sample. ## M. Performance Characteristics (if/when applicable): ### 1. Linearity/assay reportable range: The linearity was assessed by testing serial dilutions. The High Standard was prepared by adding a commercially marketed pure enzyme to an albumin matrix. Eight amylase linearity solutions were prepared by quantitatively diluting this stabilized stock standard. Linearity was assessed from duplicate testing of these solutions on the Aeroset system. The sponsor's studies show that the assay is linear to 2200 IU/L. c. The studies support the 30 day stability claim of the reagent "on board" the analyzers. d. Detection limit: The sensitivity of the assay is 1 U/L. The sensitivity is determined on twenty replicates of normal saline and represents the lowest pancreatic $\alpha$ -amylase activity that can be distinguished from zero. It is calculated as 3 SD of the twenty replicates. The company also performed a precision study with a sample value of approximately 1 U/L.
