Device Story
The Wafer System is a surgical instrument set used during proximal tibial opening wedge osteotomies. It consists of a delivery gun, a delivery track, and a wafer cartridge assembly. The system is used by surgeons in an operating room to insert PEEK wafers into the osteotomy site. These wafers act as temporary spacers to maintain the opening until bone graft and supplemental fixation are applied. The device facilitates the restoration of anatomical alignment and relative position of the tibia.

Indications for Use
Indicated for patients undergoing proximal tibial opening wedge osteotomies requiring restoration of anatomical alignment and relative position, used with supplemental fixation.

The devices contain a plate, a nail/plate combination, or a blade/plate combination that are made of alloys, such as cobalt-chromium-molybdenum, stainless steel, and titanium, that are intended to be held in position with fasteners, such as screws and nails, or bolts, nuts, and washers. These devices are used for fixation of fractures of the proximal or distal end of long bones, such as intracapsular, intertrochanteric, intercervical, supracondylar, or condylar fractures of the femur; for fusion of a joint; or for surgical procedures that involve cutting a bone. The devices may be implanted or attached through the skin so that a pulling force (traction) may be applied to the skeletal system.

## III. Device Description
The Wafer System consists of a delivery gun, a delivery track and a wafer cartridge assembly. The Wafers are used to open the osteotomy and act as a spacer until bone graft and supplemental fixation can be placed.
The Wafer System Implants are manufactured from Polyetheretherketone (PEEK) with 6% Barium Sulfate.

#### V. Substantial Equivalence
The Wafer System was demonstrated to be substantially equivalent to the Arthrex Opening Wedge Osteotomy System (K973812) and the Synthes Spiked Washer (K011583). In addition, mechanical testing demonstrated that The Wafer System met the performance requirements for its intended use. Dissimilarities between The Wafer System and the predicate devices do not affect the safety or effectiveness of this device.
