Device Story
The IMMAGE® Immunochemistry System uses rate nephelometry to quantify human IgM in serum and CSF samples. The system utilizes goat anti-human IgM antibodies to react with IgM in the sample; the resulting light scattering is measured to determine concentration. The device is intended for use in clinical laboratory settings by trained laboratory personnel. The system automates the analysis, providing quantitative results via a calibration curve, which replaces manual or semi-quantitative methods like immunofixation electrophoresis. Clinicians use these IgM concentration results to assess blood-CSF barrier integrity and identify intrathecal immunoglobulin synthesis, aiding in the diagnosis of CNS conditions such as meningitis or encephalitis.
Clinical Evidence
Bench testing only. Method comparison studies (N=55 for serum; N=51 for CSF) demonstrated high correlation with predicate methods (slope 0.977, r=0.988 for serum). CSF sensitivity was 100% and specificity ranged from 89.5% to 100%. Imprecision studies (N=80 per sample level) showed low CVs for both serum (1.7-3.5%) and CSF (2.0-6.0%) matrices.

We have reviewed your Section 510(k) notification of intent to market the device referenced above and we have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food. Drug, and Cosmetic Act (Act). You may, therefore, market the device, subject to the general controls provisions of the Act. The general controls provisions of the Act include requirements for annual registration, listing of devices, good manufacturing practice, labeling, and prohibitions against misbranding and adulteration.

Clinical Significance:
The concentration ratio of immunoglobulins in CSF and serum detects increased permeability of the blood-CSF barrier and intrathecal synthesis of immunoglobulins.
The permeability of the blood-CSF barrier to plasma increases due to brain tumor, intracerebral hemorrhage, meningitis, encephalitis, and bacterial infections. The intrathecal synthesis of immunoglobulins is important in the diagnosis of diseases of the Central Nervous Systems (CNS).
