Indications for Use

The Elevation Spine Saber-C System is a cervical interbody fusion device intended for use in skeletally mature patients with degenerative disc disease (DDD) of the cervical spine (C2-T1) and is for use in multiple contiguous spinal levels. DDD is defined as discogenic pain with degeneration of the disc confirmed by history and radiographic studies. These patients should be skeletally mature and have had at least six (6) weeks of non-operative treatment. The SABER-C Spacer is to be filled with autograft bone and/or allogenic bone graft composed of cancellous and/or corticocancellous bone. The SABER-C Spacer is intended to be used with supplemental fixation, such as anterior cervical plates or posterior cervical screw fixation. The SABER-C Spacer is available in a solid titanium and a porous 3D printed titanium offering. When the SABER-C Spacer is used with the SABER-C Anterior Cervical Plate and screws, the plate-spacer-screw assembly can be used as a stand-alone device with the SABER-C Anterior Cervical Plate acting as the supplemental fixation. When the SABER-C Spacer is used with the SABER-C Anterior Cervical Plate and spikes, the plate-spacer-spike assembly should be used with additional supplemental fixation such as posterior cervical screw fixation. The SABER-C Anterior Cervical Plate, when used without the spacer component and with screws, is intended for anterior screw fixation to the cervical spine (C2-C7) for the following indications: degenerative disc disease (as defined by neck pain of discogenic origin with degeneration of the disc confirmed by patient history and radiographic studies), trauma (including fractures), tumors, deformity (defined as kyphosis, lordosis, or scoliosis), pseudoarthrosis, failed previous fusion, spondylolisthesis, and spinal stenosis. The Anterior Cervical Plate is not to be used with spikes when used without the Spacer component.

Device Story

SABER-C System is an anterior cervical interbody fusion device providing structural stability post-discectomy. System includes interbody spacers (solid titanium or porous 3D-printed titanium) and anterior cervical plates with spike or screw fixation. Spacers are filled with autograft or allogenic bone graft. Used in OR by surgeons; inserted via anterior cervical approach. Plate-spacer-screw assembly functions as stand-alone device; plate-spacer-spike assembly requires additional posterior fixation. Plate alone (with screws) treats DDD, trauma, tumors, deformity, pseudoarthrosis, failed fusion, spondylolisthesis, and spinal stenosis. Device provides mechanical support to facilitate spinal fusion; benefits patient by stabilizing cervical spine segments.

Clinical Evidence

Bench testing only. Previously submitted testing in K260660 includes static and dynamic axial compression, compression shear, and torsion (ASTM F2077), as well as expulsion and subsidence (ASTM F2267).

Technological Characteristics

Materials: radiolucent polymer (ASTM F2026), titanium alloy (ASTM F136, F1295), tantalum markers (ASTM F560), and porous 3D-printed titanium (ASTM F3001). Footprints: 12x15mm, 14x15mm, 14x17mm, 15x18mm. Heights: 5-10mm. Lordotic options: 0°, 6°, 9°, 12°. Fixation: spikes or screws. Prescription use.

Indications for Use

Indicated for skeletally mature patients with degenerative disc disease (DDD) of the cervical spine (C2-T1) at multiple contiguous levels. Also indicated for anterior screw fixation (C2-C7) for DDD, trauma, tumors, deformity, pseudoarthrosis, failed fusion, spondylolisthesis, and spinal stenosis. Requires 6 weeks of non-operative treatment.

Regulatory Classification

Identification

An intervertebral body fusion device is an implanted single or multiple component spinal device made from a variety of materials, including titanium and polymers. The device is inserted into the intervertebral body space of the cervical or lumbosacral spine, and is intended for intervertebral body fusion.

Special Controls

*Classification.* (1) Class II (special controls) for intervertebral body fusion devices that contain bone grafting material. The special control is the FDA guidance document entitled “Class II Special Controls Guidance Document: Intervertebral Body Fusion Device.” See § 888.1(e) for the availability of this guidance document.(2) Class III (premarket approval) for intervertebral body fusion devices that include any therapeutic biologic (e.g., bone morphogenic protein). Intervertebral body fusion devices that contain any therapeutic biologic require premarket approval. (c) *Date premarket approval application (PMA) or notice of product development protocol (PDP) is required.* Devices described in paragraph (b)(2) of this section shall have an approved PMA or a declared completed PDP in effect before being placed in commercial distribution.

The only difference between the subject and primary predicate is the expansion of the indications for use to include use at multiple contiguous levels, similar to the additional predicate. # **Performance Testing:** The following bench performance testing was previously performed on the subject additively manufactured interbody devices and submitted in K260660 and remains applicable to this submission: Static and Dynamic Axial Compression, Static and Dynamic Compression Shear, Static and Dynamic Torsion per ASTM F2077, Expulsion, and Subsidence per ASTM F2267. # **Conclusion:** The subject device is identical in every way to the predicate (K260660). No new worst case is introduced. The expansion of the indications for use does not create new risks that are not mitigated by the previously performed testing. Therefore, no additional testing is needed.
