Device Story
Xpert Ankle is a system of pre-contoured metallic bone plates and screws; used for internal fixation of fractures, osteotomies, and pseudarthroses of the distal/diaphyseal fibula and distal tibia, and for syndesmotic repair. Implants are provided either sterile (gamma) or non-sterile for healthcare professional sterilization. System includes associated surgical instruments. Surgeons use these implants in an operating room setting to stabilize bone segments; mechanical fixation promotes bone healing. Device is intended for adult patients.
Clinical Evidence
No clinical studies were performed. Substantial equivalence is supported by non-clinical bench testing, including static and dynamic fatigue testing of plates (ASTM F382), and torsional strength, driving torque, and pullout strength testing of screws (ASTM F543).
Technological Characteristics
Plates: CP Titanium grade 4 (ASTM F67, ISO 5832-2). Screws: Titanium alloy Ti-6Al-4V ELI (ASTM F136, ISO 5832-3). System consists of pre-contoured plates and screws for bone fixation. Sterilization: Gamma radiation or healthcare facility steam sterilization. No software or electronic components.

Indicated for fixation of fractures, osteotomies, and pseudarthroses of the distal and diaphyseal fibula, distal tibia, and for syndesmotic repair in adults (22 years and older).

The devices contain a plate, a nail/plate combination, or a blade/plate combination that are made of alloys, such as cobalt-chromium-molybdenum, stainless steel, and titanium, that are intended to be held in position with fasteners, such as screws and nails, or bolts, nuts, and washers. These devices are used for fixation of fractures of the proximal or distal end of long bones, such as intracapsular, intertrochanteric, intercervical, supracondylar, or condylar fractures of the femur; for fusion of a joint; or for surgical procedures that involve cutting a bone. The devices may be implanted or attached through the skin so that a pulling force (traction) may be applied to the skeletal system.

The Xpert Ankle range consists of pre-contoured tibial and fibula plates and screws in various sizes designed for the fixation of fractures, osteotomies and pseudarthroses of the distal and the diaphyseal fibula, the distal tibia and for the syndesmotic repair in adults.
The plates and screws of the Xpert Ankle range will be provided sterile by gamma sterilization or nonsterile for sterilization by health care professionals prior to use.
The instruments of the Xpert Ankle will be provided nonsterile for sterilization by health care professionals prior to use.
Single use kits (Initial A Xpert) contain implants and instruments provided sterile by gamma sterilization.

# 6. Summary of the technological characteristics compared to predicate
The Xpert Ankle range is substantially equivalent to the predicate devices in intended use, design, manufactured materials, and mechanical performance.
The following tests and analyses were conducted:
- Engineering analysis (static) and dynamic fatigue testing were performed on the worst-case plate in accordance with ASTM F382, with the results compared to the Arthrex Fracture Plates (K151732) predicate device.
- Torsional strength and driving torque were performed on the worst-case screw in accordance with ASTM F543, with the results evaluated against the minimum performance criteria specified in FDA's Guidance document, Orthopedic Non-Spinal Metallic Bone Screws and Washers – Performance Criteria for Safety and Performance Based Pathway (2020).
- Pullout strength was evaluated through engineering analysis using the Chapman (1996) equation, as specified in the FDA Guidance, with the results evaluated against the guidance's minimum acceptable values.
