Intended Use
The Syndesmosis TightRope PRO is intended as an adjunct in fracture repair involving metaphyseal and periarticular small bone fragments where screws are not indicated, and as an adjunct in external and intramedullary fixation systems involving plates and rods, with fracture braces and casting. Specifically, the Syndesmosis TightRope PRO is intended to provide fixation during the healing process following a syndesmotic trauma, such as fixation of syndesmosis (syndesmosis disruptions) in connection with Weber B and C ankle fractures.
Device Story
Syndesmosis TightRope PRO is a metallic bone fixation system; consists of two metal buttons (round/oblong) preloaded onto an inserter with #5 suture and #2-0 FiberWire passing suture; includes one-hole washer or two-hole buttress plate. Used by surgeons in clinical settings for syndesmotic trauma fixation. Device provides mechanical stabilization during healing process; serves as alternative to screws. Output is physical fixation of bone fragments. Benefits include stable syndesmotic repair in ankle fractures.
Clinical Evidence
Bench testing only. Performance demonstrated via cyclic shear testing and ultimate shear testing compared to predicate devices. MRI safety (force, torque, image artifact, RF heating) evaluated per ASTM F2052, F2119, F2182, and F2213 standards.
Technological Characteristics
Metallic bone fixation appliance. Components: metal buttons, #5 suture, #2-0 FiberWire, one-hole washer, or two-hole buttress plate. MRI compatible. Sterilized device. Design features include specific button configurations and drill bit diameters.
Indications for Use
Indicated for patients requiring fixation during healing of syndesmotic trauma, including syndesmosis disruptions associated with Weber B and C ankle fractures, and as an adjunct in fracture repair of metaphyseal/periarticular small bone fragments or with external/intramedullary fixation systems.

Single/multiple component metallic bone fixation appliances and accessories are devices intended to be implanted consisting of one or more metallic components and their metallic fasteners. The devices contain a plate, a nail/plate combination, or a blade/plate combination that are made of alloys, such as cobalt-chromium-molybdenum, stainless steel, and titanium, that are intended to be held in position with fasteners, such as screws and nails, or bolts, nuts, and washers. These devices are used for fixation of fractures of the proximal or distal end of long bones, such as intracapsular, intertrochanteric, intercervical, supracondylar, or condylar fractures of the femur; for fusion of a joint; or for surgical procedures that involve cutting a bone. The devices may be implanted or attached through the skin so that a pulling force (traction) may be applied to the skeletal system.

We have reviewed your section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (the Act) that do not require approval of a premarket approval application (PMA). You may, therefore, market the device, subject to the general controls provisions of the Act. Although this letter refers to your product as a device, please be aware that some cleared products may instead be combination products. The 510(k) Premarket Notification Database available at https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfpmn/pmn.cfm identifies combination product submissions. MRI force, torque, and image artifact testing was previously conducted in accordance with FDA guidance Testing and Labeling Medical Devices for Safety in the Magnetic Resonance (MR) Environment, ASTM F2052 Standard Test Method for Measurement of Magnetically Induced Displacement Force on Medical Devices in the Magnetic Resonance Environment, ASTM F2119 Standard Test Method for Evaluation of MR Image Artifacts from Passive Implants, ASTM F2182 Standard Test Method for Measurement of Measurement of Radio Frequency Induced Heating Near Passive Implants During Magnetic Resonance Imaging and ASTM F2213 Standard Test Method for Measurement of Magnetically Induced Torque on Medical Devices in the Magnetic Resonance Environment. Arthrex determined that the proposed Syndesmosis TightRope PRO does not represent a new worst-case in terms of MR compatibility and will be adopted into the MRI labeling previously reviewed and accepted by FDA within additional predicate device, Arthrex Syndesmosis TightRope XP Buttress Plate Implant System (K201522) and Arthrex ACL TightRope, PCL TightRope, and TightRope II (K221128).
Assessment of the physical product attributes including product, design, size, and materials has determined that the Syndesmosis TightRope PRO does not introduce additional risks or concerns regarding sterilization and shelf-life. |

Any differences between the proposed and predicate devices are considered minor and do not raise different questions concerning safety and effectiveness. |
