Insignia Hip Stem and Restoration Modular 115mm Conical Distal Stem are orthopedic implants for total or hemi hip arthroplasty. Insignia stems are cementless, single-use devices for femoral canal fixation; Restoration Modular stems are press-fit, modular components for femoral replacement. Devices are implanted by orthopedic surgeons in clinical/OR settings. Implants restore hip biomechanics and alleviate pain. This submission updates labeling to include MR Conditional status. No software or algorithms are involved.

No clinical data. Substantial equivalence is based on design, material, and intended use comparisons, supported by non-clinical bench testing for MR compatibility (ASTM F2052-15, F2213-17, F2119-07, F2182-19).
Technological Characteristics
Insignia Hip Stem: Ti6Al4V ELI alloy, hydroxyapatite coating, titanium plasma sprayed surface. Restoration Modular 115mm Conical Distal Stem: Modular femoral replacement system with male locking taper. Both are sterile, single-use, cementless/press-fit implants. MR Conditional.
Indications for Use
Indicated for patients with painful, disabling hip joint disease (noninflammatory degenerative joint disease, osteoarthritis, avascular necrosis, rheumatoid arthritis, post-traumatic arthritis), functional deformity, nonunion, femoral neck/trochanteric fractures, or revision of failed previous procedures. Insignia Hip Stems are indicated for primary/revision total or hemiarthroplasty; specific configurations are indicated for patients at high risk of dislocation (history of dislocation, bone loss, soft tissue laxity, neuromuscular disease, intra-operative instability). Restoration Modular 115mm Conical Distal Stem is indicated for primary/revision total hip arthroplasty, including cases with severe proximal bone loss.
Regulatory Classification
Identification
A hip joint metal/ceramic/polymer semi-constrained cemented or nonporous uncemented prosthesis is a device intended to be implanted to replace a hip joint. This device limits translation and rotation in one or more planes via the geometry of its articulating surfaces. It has no linkage across-the-joint. The two-part femoral component consists of a femoral stem made of alloys to be fixed in the intramedullary canal of the femur by impaction with or without use of bone cement. The proximal end of the femoral stem is tapered with a surface that ensures positive locking with the spherical ceramic (aluminium oxide, A12 03 ) head of the femoral component. The acetabular component is made of ultra-high molecular weight polyethylene or ultra-high molecular weight polyethylene reinforced with nonporous metal alloys, and used with or without bone cement.

The subject Restoration Modular 115mm Conical Distal Stem is a part of the Restoration Modular Hip System. The Restoration Modular Hip System is a modular femoral replacement system comprised of three main components: the Restoration Modular Proximal Femoral Body, the Restoration Modular Distal Stem, and the Locking Bolt. Each of these components feature unique design iterations and are offered in a range of sizes, allowing for independent sizing of the proximal femoral body and the distal stem. The distal stem addresses fixation and stability, and the proximal femoral geometry allows for adjustments to anteversion, height, and offset. This premarket notification presents an evaluation to characterize the components of the Insignia Hip Stem System as with respect to magnetically induced displacement force, magnetically induced rotational force, MR-induced image artifacts, and R-F induced heating. The subject Insignia Hip Stem components were evaluated per the following standards:

Select the rows, go to the Collections tab, and create a labeled collection (e.g., "Cobb Angle Project"). You can reload the selection anytime and carry it into the PDF viewer and other tabs that support selections.
7. Product codes and the regulations tree
Click a product code in the results to jump to it in the regulations tree — identification text, sibling product codes, and devices you can open in a PDF viewer on the right. Click a regulation number to see its identification, special controls, and related product codes. You can also search by product code or regulation number at the top of the tree. Always read the special controls if any exist for your device — it broadens your search and sharpens pre-kickoff research.

8. Chart view
Click Show Chart and segment by regulation number (or product code) to see which regulations dominate your result set. Clicking a regulation takes you into the regulations tree. Great for spotting that most matches are, say, hardware laparoscopic devices — a cue to go back and filter.

9. The predicate graph
Open the Predicates tab for a family-tree view of predicate relationships. Click a node to trace its parents and children; selections from search carry over pre-selected. The visual lineage is also handy on client calls, e.g. to show how a predicate family evolved and justify why your predicate still holds.

10. Embeddings: the galaxy map
The Embeddings tab plots every matching document in a 2-D "galaxy map" where semantically similar devices cluster together. Hover or click clusters to explore, and let AI label the clusters for you. Embeddings beat product codes for grouping: two devices can carry different product codes (LLZ vs. QIH) yet do the same thing — the embedding captures the meaning of the intended use and device story. This is also exactly how retrieval-augmented generation (RAG) works under the hood, and it makes a great visual on client calls.
