Intended Use

The Hexanium® TLIF (Transforaminal Lumbar Interbody Fusion) system is an intervertebral body fusion device indicated for use with autogenous bone graft in skeletally mature patients with Degenerative Disc Discase (DDD) at one or two continuous levels from L2-S1. DDD is defined as discogenic back pain with degeneration of the disc confirmed by patient history and radiographic studies. These DDD patients may also have up to Grade I spondylolisthesis or retrolisthesis at the involved level(s). Patients should have received at least 6 months of non-operative treatment prior to treatment with Hexanium® TLIF system. This device has to be filled with autogenous bone graft material. This device is implanted via transforaminal approach. Hexanium® TLIF system must be used in combination with supplemental internal spinal fixation which has been cleared by the FDA for use in the lumbar spine.

Device Story

Hexanium® TLIF is a titanium alloy (Ti6Al4V ELI) interbody cage produced via additive manufacturing; features honeycomb structure for bone through-growth and graft windows. Implanted via transforaminal approach in lumbar spine (L2-S1) to facilitate fusion; must be filled with autogenous bone graft and used with supplemental internal spinal fixation. Used by surgeons in clinical settings to treat discogenic back pain associated with DDD. Device provides structural support to intervertebral space; honeycomb design promotes osseointegration. Benefits include stabilization of spinal segment and potential reduction of pain.

Clinical Evidence

No clinical data has been presented. Evidence is based on bench testing (mechanical testing per ASTM F2077-14 and ASTM 2267-04) and FEA analysis comparing the subject device to the predicate worst-case construct.

Technological Characteristics

Material: Titanium alloy (Ti6Al4V ELI). Manufacturing: Additive manufacturing. Design: Honeycomb structure with bone graft windows; 5° lordosis angle. Dimensions: Heights 7-16mm; footprints 28x9mm to 32x10.5mm. Sterilization: Provided sterile. Standards: ASTM F2077-14, ASTM 2267-04, USP bacterial endotoxins.

Indications for Use

Indicated for skeletally mature patients with Degenerative Disc Disease (DDD) at one or two continuous levels from L2-S1, including those with up to Grade I spondylolisthesis or retrolisthesis, who have failed at least 6 months of non-operative treatment.

An intervertebral body fusion device is an implanted single or multiple component spinal device made from a variety of materials, including titanium and polymers. The device is inserted into the intervertebral body space of the cervical or lumbosacral spine, and is intended for intervertebral body fusion.

Special Controls

*Classification.* (1) Class II (special controls) for intervertebral body fusion devices that contain bone grafting material. The special control is the FDA guidance document entitled “Class II Special Controls Guidance Document: Intervertebral Body Fusion Device.” See § 888.1(e) for the availability of this guidance document.(2) Class III (premarket approval) for intervertebral body fusion devices that include any therapeutic biologic (e.g., bone morphogenic protein). Intervertebral body fusion devices that contain any therapeutic biologic require premarket approval. (c) *Date premarket approval application (PMA) or notice of product development protocol (PDP) is required.* Devices described in paragraph (b)(2) of this section shall have an approved PMA or a declared completed PDP in effect before being placed in commercial distribution.
