Intended Use

The XL Dental Implant System is intended for implantation in the maxillary or mandibular molar region where bone exists and the surgeon has determined that the placement of a narrower diameter implant would increase the probability of failure due to poor primary stability, or increased surgical procedures leading to complications. This XL implant system provides support for fixed or removable dental prostheses in a single tooth, partially edentulous prostheses or full arch prostheses. It further adds the option for immediate loading on single and splinted multiple unit restorations when good primary stability is achieved and with appropriate occlusal loading, to restore chewing function.

Device Story

The Keystone Dental XL Dental Implant System consists of threaded root-form dental implants and associated prosthetic components (cover screws, healing abutments, titanium cylinder/permanent abutments, abutment screws). Designed for the molar region, the system provides structural support for dental prostheses. Implants are placed surgically by a clinician into the maxillary or mandibular bone. The device functions as an anchor for fixed or removable restorations, enabling immediate loading when primary stability is achieved. The system restores chewing function for patients with missing teeth. The device is used in a clinical setting by dental professionals.

Clinical Evidence

Bench testing only. Evidence includes biocompatibility evaluation per ISO 10993-1, bacterial endotoxin (LAL) testing meeting <20 EU/device, and sterilization validation per ISO 11137-1/2. Surface analysis and packaging/sterilization processes were validated by referencing predicate device K112795.

Technological Characteristics

Root-form endosseous dental implants; diameters 7, 8, 9mm; lengths 7, 9, 11mm. Material: Titanium alloy (ASTM F136). Surface: Grit-blasted, acid-etched (SLA). Connection: Internal hex. Sterilization: Gamma radiation. Components include cover screws, healing abutments, and titanium abutments.

Indications for Use

Indicated for patients requiring dental implants in the maxillary or mandibular molar region where bone is present, specifically when narrower implants risk failure due to poor primary stability or surgical complications. Supports fixed or removable prostheses (single, partial, or full arch) with options for immediate loading.

Regulatory Classification

Identification

An endosseous dental implant is a prescription device made of a material such as titanium or titanium alloy that is intended to be surgically placed in the bone of the upper or lower jaw arches to provide support for prosthetic devices, such as artificial teeth, in order to restore a patient's chewing function.

Special Controls

*Classification.* (1) Class II (special controls). The device is classified as class II if it is a root-form endosseous dental implant. The root-form endosseous dental implant is characterized by four geometrically distinct types: Basket, screw, solid cylinder, and hollow cylinder. The guidance document entitled “Class II Special Controls Guidance Document: Root-Form Endosseous Dental Implants and Endosseous Dental Implant Abutments” will serve as the special control. (See § 872.1(e) for the availability of this guidance document.)(2) *Classification.* Class II (special controls). The device is classified as class II if it is a blade-form endosseous dental implant. The special controls for this device are:(i) The design characteristics of the device must ensure that the geometry and material composition are consistent with the intended use; (ii) Mechanical performance (fatigue) testing under simulated physiological conditions to demonstrate maximum load (endurance limit) when the device is subjected to compressive and shear loads; (iii) Corrosion testing under simulated physiological conditions to demonstrate corrosion potential of each metal or alloy, couple potential for an assembled dissimilar metal implant system, and corrosion rate for an assembled dissimilar metal implant system; (iv) The device must be demonstrated to be biocompatible; (v) Sterility testing must demonstrate the sterility of the device; (vi) Performance testing to evaluate the compatibility of the device in a magnetic resonance (MR) environment; (vii) Labeling must include a clear description of the technological features, how the device should be used in patients, detailed surgical protocol and restoration procedures, relevant precautions and warnings based on the clinical use of the device, and qualifications and training requirements for device users including technicians and clinicians; (viii) Patient labeling must contain a description of how the device works, how the device is placed, how the patient needs to care for the implant, possible adverse events and how to report any complications; and (ix) Documented clinical experience must demonstrate safe and effective use and capture any adverse events observed during clinical use.

The subject device implants have the same design as the implants in K071161 and K191054 with the use of Titanium Alloy in accordance with ASTM F136 as the implant material. This Titanium Alloy material is common in the dental implant industry and the same as is used in other predicate devices. The subject device has the same material, surface, internal hex connection and packaging as the reference device K112795, Paltop Dental Implants (a Keystone Dental Group company). The subject device has the same abutment design as the reference predicate device K101565. Like the predicate devices, the subject device components are for single use and are provided sterile. Substantial equivalence of the subject device components in terms of biocompatibility is supported by the fact that materials are identical in formulation. processing, and component interfaces to reference devices K112795. #### CONCLUSION The subject device and the predicate and reference devices have the same intended use, technological characteristics, and materials. The subject device and the predicate and reference devices encompass the same range of physical dimensions, including diameter and length of the implants, and the design of the abutments. The subject device and the reference predicate devices are packaged in the same materials and sterilized using the same method and processes. The data included in this submission demonstrate substantial equivalence to the predicate and reference devices listed above.
