Intended Use
The Ascendant™ Cervical Spacer System is indicated for anterior cervical interbody fusion procedures in skeletally mature patients with degenerative disc disease at one disc level from C2-T1. Degenerative Disc Disease (DDD) is defined as discogenic pain with degeneration of the disc confirmed by history and radiographic studies. These patients should have had six weeks of non-operative treatment. The Ascendant™ Cervical Spacer System is to be used with autogenous bone graft and supplemental fixation (i.e., an anterior cervical plate), and is implanted via an open, anterior approach. Patients with previous non-fusion spinal surgery at involved level may be treated with the device.
Device Story
The Ascendant™ Cervical Spacer System is an intervertebral body fusion device designed for anterior cervical spine surgery. The device consists of a PEEK Optima® LT1 cage featuring a CP titanium coating and tantalum radiographic markers. It includes an internal chamber for autogenous bone graft material. The system is provided in various heights, footprints, and lordotic angles to match patient anatomy. It is implanted by a surgeon via an open, anterior approach and requires supplemental fixation using an anterior cervical plate. The device acts as a structural spacer to facilitate fusion between vertebral bodies. It is intended for use in clinical settings where spinal fusion is indicated.
Clinical Evidence
Bench testing only. Testing included static and dynamic compressive shear, static expulsion, dynamic axial compression, dynamic torsion, percent porosity, coating thickness, static shear, static tensile, shear fatigue, and abrasion testing per ASTM standards (F2077, F1854-09, F1044-05, F1147-05, F1160-05, F1978-00).
Technological Characteristics
Materials: PEEK Optima® LT1, CP titanium coating, tantalum markers. Form factor: Interbody cage with enclosed graft chamber, various heights/footprints/lordotic angles. Standards: ASTM F2077 (mechanical), ASTM F1854-09 (porosity/coating), ASTM F1044-05 (shear), ASTM F1147-05 (tensile), ASTM F1160-05 (fatigue), ASTM F1978-00 (abrasion).
Indications for Use
Indicated for anterior cervical interbody fusion in skeletally mature patients with degenerative disc disease (DDD) at one level (C2-T1) failing 6 weeks of non-operative treatment. Includes patients with prior non-fusion surgery at the level. Must be used with autogenous bone graft and supplemental anterior cervical plate fixation.

Identification
An intervertebral body fusion device is an implanted single or multiple component spinal device made from a variety of materials, including titanium and polymers. The device is inserted into the intervertebral body space of the cervical or lumbosacral spine, and is intended for intervertebral body fusion.
Special Controls
*Classification.* (1) Class II (special controls) for intervertebral body fusion devices that contain bone grafting material. The special control is the FDA guidance document entitled “Class II Special Controls Guidance Document: Intervertebral Body Fusion Device.” See § 888.1(e) for the availability of this guidance document.(2) Class III (premarket approval) for intervertebral body fusion devices that include any therapeutic biologic (e.g., bone morphogenic protein). Intervertebral body fusion devices that contain any therapeutic biologic require premarket approval.

The Ascendant™ Cervical Spacer System is an anterior cervical interbody device consisting of a PEEK Optima® LT1 (polyetheretherkeytone) implant cage with CP titanium coating and tantalum radiographic markers. It is intended for use as an interbody fusion device and is offered in a variety of heights, footprints and lordotic angles to accommodate varying anatomical conditions. The device features an enclosed chamber intended to be filled with autogenous bone graft material.
The Ascendant™ Cervical Spacer System is intended to be used with supplemental fixation (i.e., an anterior cervical plate).
#### INDICATIONS FOR USE
The Ascendant™ Cervical Spacer System is indicated for anterior cervical interbody fusion procedures in skeletally mature patients with degenerative disc disease at one disc level from C2-T1. Degenerative Disc Disease (DDD) is defined as discogenic pain with degeneration of the disc confirmed by history and radiographic studies. These patients should have had six weeks of non-operative treatment. The Ascendant™ Cervical Spacer System is to be used with autogenous bone graft and supplemental fixation (i.e., an anterior cervical plate), and is implanted via an open, anterior approach. #### CONCLUSION
The overall technology characteristics and mechanical performance data lead to the conclusion that the Ascendant™ Cervical Spacer System, Ti Coated is substantially equivalent to the predicate device.
