Device Story

Sternal Plating System provides rigid bone fixation for sternal fragments. Device consists of titanium alloy (Ti-6Al-4V) plates (1.0mm–3.0mm thickness) and bone screws (2.3mm–3.2mm diameter). Plates feature elongated midsection to facilitate re-entry during subsequent thoracic procedures. Used by surgeons in clinical/OR settings to stabilize sternum post-sternotomy or for reconstruction. System provides greater stiffness and lower lateral displacement compared to traditional stainless steel cerclage wire fixation, potentially improving sternal stability and healing outcomes.

Clinical Evidence

Evidence includes biomechanical study (Ozaki et al.) comparing prototype titanium 'H' plates to stainless steel cerclage wires in human cadavers. Results showed greater stiffness and lower lateral displacement for rigid titanium plates. Additional support provided by clinical literature (Sargent et al., Cheng et al., Buchman et al.) demonstrating superiority of plating systems over traditional sternal wiring.

Single/multiple component metallic bone fixation appliances and accessories are devices intended to be implanted consisting of one or more metallic components and their metallic fasteners. The devices contain a plate, a nail/plate combination, or a blade/plate combination that are made of alloys, such as cobalt-chromium-molybdenum, stainless steel, and titanium, that are intended to be held in position with fasteners, such as screws and nails, or bolts, nuts, and washers. These devices are used for fixation of fractures of the proximal or distal end of long bones, such as intracapsular, intertrochanteric, intercervical, supracondylar, or condylar fractures of the femur; for fusion of a joint; or for surgical procedures that involve cutting a bone. The devices may be implanted or attached through the skin so that a pulling force (traction) may be applied to the skeletal system.

The function of these systems is to affix bonv sternum fragments. The KLS-Martin Sternal Plating System uses previously cleared Titanlum Allov (TI-6 Al-4V) screws to affix the plates to the stemum. The plate design includes an elongated midsection to facilitate quick re-entry in subsequent thoracic procedures. The plates and screws are substantially equivalent in strength to the Lorenz Sternal Closure System with Modular Screws (K011079). ## Differences to Predicate The KLS-Martin Sternal Plating System is the same as the Lorenz Stemal Closure System with Modular Screws (K011079) with the exception that the screw is a standard bone screw and not a modular screw. The KLS-Martin Sternal System is substantially equivalent Substantial Equivalence: in application and function to the Lorenz Sternal Closure System with Modular Screw and Ethicon Surgical Stainless Steel Suture, U.S.P. sizes 5 to 7. > Substantial equivalence is based on comparison of performance, method of rigid bone fixation and clinical literature assessment. > Intended use of the KLS-Martin Stemal System is the same as the Lorenz Stemal Closure System with Modular Screw; the function of this system being to affix Stemal bony fragments. > Superiority of Sternal Plating to sternum wiring has been demonstrated in clinical studies. Support of this statement includes studies by Sargent et al., Cheng et al., Buchman et al., and Ozaki et al. {2}------------------------------------------------ Clinical/Non-clinical Tests: The three clinical studies used to support the Statement of Superiority of Plating Systems are by Sargent et al., Cheng et al. and Buchman et al.
