Intended Use

The ORTHOLOC™ SPS is intended for fractures and fracture dislocations, osteotomies, and non-unions of the proximal humerus, particularly in osteopenic bone.

Device Story

ORTHOLOC™ SPS Shoulder Plating System provides anatomical reduction and stable primary fixation of proximal humerus. System comprises stainless steel plates (Standard, Posterior, Greater Tuberosity designs) and locking/non-locking screws. Used by surgeons in clinical settings for bone fixation. Device requires steam sterilization prior to use. Modifications include addition of blunt-tipped locking screws and alternative stainless-steel alloy for cortical non-locking screws. Device functions via mechanical fixation of bone segments to facilitate healing.

Clinical Evidence

No clinical studies were performed. Substantial equivalence supported by non-clinical bench testing, including screw torsion, driving torque, and axial pullout per ASTM F543-17.

Technological Characteristics

Metallic bone fixation system. Plates: stainless steel (ASTM F138). Locking screws: stainless steel (ASTM F2229). Non-locking screws: stainless steel (ASTM F138 or ASTM F2229). Mechanical fixation principle. Non-sterile; requires steam sterilization.

Indications for Use

Indicated for patients with fractures, fracture dislocations, osteotomies, and non-unions of the proximal humerus, including those with osteopenic bone.

Single/multiple component metallic bone fixation appliances and accessories are devices intended to be implanted consisting of one or more metallic components and their metallic fasteners. The devices contain a plate, a nail/plate combination, or a blade/plate combination that are made of alloys, such as cobalt-chromium-molybdenum, stainless steel, and titanium, that are intended to be held in position with fasteners, such as screws and nails, or bolts, nuts, and washers. These devices are used for fixation of fractures of the proximal or distal end of long bones, such as intracapsular, intertrochanteric, intercervical, supracondylar, or condylar fractures of the femur; for fusion of a joint; or for surgical procedures that involve cutting a bone. The devices may be implanted or attached through the skin so that a pulling force (traction) may be applied to the skeletal system.

The letter "T" in the hexagon has a smaller hexagon inside of it. ## Comparison of Technological Characteristics with the Predicate Device The modifications to the predicate system include the addition of blunt-tipped locking screws and an alternative stainless-steel alloy (ASTM F2229) for cortical non-locking screws. #### Non-clinical Performance Testing To demonstrate substantial equivalence to the predicate device, the following non-clinical bench testing was performed as indicated by risk analysis: - Screw Torsion, Driving Torque, and Axial Pullout to ASTM F543-17 ● The subject device passed all testing with the same acceptance criteria as the predicate device. ## Clinical Testing No clinical studies were performed. #### Conclusions The subject device is identical to the predicate device with respect to intended use, principle of operation, and conditions for use. The ORTHOLOC™ SPS Shoulder Plating System does not raise new questions of safety or effectiveness. Differences in technological characteristics that pose potential risks, as identified by risk analysis, have been addressed with verification testing. The results of testing for the modified ORTHOLOC™ SPS Shoulder Plating System support substantial equivalence to the predicate ORTHOLOC™ SPS Shoulder Plating System (K181587).
