Intended Use

FORZA Ti Spacer System is indicated for spinal fusion procedures in skeletally mature patients with degenerative disc disease (DDD) at one or two contiguous levels in the lumbar spine (L2-S1). DDD is defined as back pain of discogenic origin with degeneration of the disc confirmed by patient history and radiographic studies. DDD patients may also have up to Grade 1 spondvlolisthesis at the involved levels. These patients may have had a previous non-fusion surgery at the involved levels. FORZA Ti Spacer System is intended for use with autograft and/or allograft comprised of cancellous and/or corticocancellous bone graft and supplemental fixation system. Patients must have undergone a regimen of at least six months of non-operative treatment prior to being treated with the FORZA Ti Spacer System.

Device Story

FORZA Ti Spacer System consists of 3D-printed titanium intervertebral body fusion implants; designed for insertion into lumbar disc space to stabilize vertebrae, maintain disc height, and prevent vertebral collapse. Implants feature porous endplates with functional gradient structure, open macroscopic pores, and microscopic/nano-scale surface features. Used in spinal fusion procedures by surgeons; requires supplemental fixation and bone graft (autograft/allograft). Device facilitates fusion by providing structural support during healing. Benefits include stabilization of spinal segments in patients with degenerative disc disease.

Clinical Evidence

Bench testing only. Mechanical testing performed per ASTM F2077 (static/dynamic axial compression, static/dynamic compression shear) and ASTM F2267 (subsidence). Particle characterization performed per ASTM F1877. Results demonstrate performance equivalent to predicate devices.

Technological Characteristics

3D-printed titanium implants conforming to ASTM F3001. Features porous endplates, functional gradient porous structure, and nano-scale surface features. Intervertebral body fusion device. Provided sterile. No software or electronic components.

Indications for Use

Indicated for spinal fusion in skeletally mature patients with degenerative disc disease (DDD) at one or two contiguous lumbar levels (L2-S1). Includes patients with up to Grade I spondylolisthesis and those with prior non-fusion surgery. Requires 6 months of failed non-operative treatment. Must be used with autograft/allograft and supplemental fixation.

The device is inserted into the intervertebral body space of the cervical or lumbosacral spine, and is intended for intervertebral body fusion.

Special Controls

The special control is the FDA guidance document entitled “Class II Special Controls Guidance Document: Intervertebral Body Fusion Device.” See § 888.1(e) for the availability of this guidance document.(2) Class III (premarket approval) for intervertebral body fusion devices that include any therapeutic biologic (e.g., bone morphogenic protein). Intervertebral body fusion devices that contain any therapeutic biologic require premarket approval. (c) *Date premarket approval application (PMA) or notice of product development protocol (PDP) is required.* Devices described in paragraph (b)(2) of this section shall have an approved PMA or a declared completed PDP in effect before being placed in commercial distribution.
