Intended Use

The Genesys Spine Apache® Interbody Fusion System is indicated for intervertebral body fusion of the lumbar spine, from L2 to SI, in skeletally mature patients who had six months of non-operative treatment. The device is intended for use at either one level or two contiguous levels for the treatment of degenerative disc disease (DDD) with up to Grade I spondylolisthesis or retrolisthesis at the involved level(s). DDD is defined as back pain of discogenic origin with degeneration of the disc confirmed by history and radiographic studies. The device system is designed for use with supplemental fixation and with autograft to facilitate fusion.

Device Story

The Genesys Spine Apache Interbody Fusion System consists of interbody fusion implants designed for lumbar spine stabilization. Implants are inserted via posterior (PLIF), transforaminal (TLIF), anterior/anterolateral (ALIF), or lateral (LLIF) surgical approaches to treat degenerative disc disease. The device acts as a structural spacer to facilitate fusion when used with autograft and supplemental fixation. It is intended for use by orthopedic or neurosurgeons in a hospital/surgical setting. The system provides various configurations to accommodate patient anatomy. Clinical benefit is derived from stabilizing the spinal segment to alleviate discogenic back pain.

Clinical Evidence

No clinical data. Substantial equivalence is supported by bench testing, specifically Finite Element Analysis (FEA), cross-sectional area calculations, and projected area calculations, which were validated against the mechanical test data of the predicate system.

Technological Characteristics

Implants manufactured from medical grade Ti-6Al-4V ELI titanium alloy per ASTM F136. System includes various configurations for PLIF, TLIF, ALIF, and LLIF surgical approaches. Mechanical performance evaluated via FEA and geometric calculations.

Indications for Use

Indicated for intervertebral body fusion of the lumbar spine (L2-S1) in skeletally mature patients with degenerative disc disease (DDD) and up to Grade I spondylolisthesis or retrolisthesis, following six months of failed non-operative treatment. Requires supplemental fixation and autograft.

An intervertebral body fusion device is an implanted single or multiple component spinal device made from a variety of materials, including titanium and polymers. The device is inserted into the intervertebral body space of the cervical or lumbosacral spine, and is intended for intervertebral body fusion.

Special Controls

*Classification.* (1) Class II (special controls) for intervertebral body fusion devices that contain bone grafting material. The special control is the FDA guidance document entitled “Class II Special Controls Guidance Document: Intervertebral Body Fusion Device.” See § 888.1(e) for the availability of this guidance document.(2) Class III (premarket approval) for intervertebral body fusion devices that include any therapeutic biologic (e.g., bone morphogenic protein). Intervertebral body fusion devices that contain any therapeutic biologic require premarket approval. (c) *Date premarket approval application (PMA) or notice of product development protocol (PDP) is required.* Devices described in paragraph (b)(2) of this section shall have an approved PMA or a declared completed PDP in effect before being placed in commercial distribution.

The Genesys Spine Apache Interbody Fusion System implants are manufactured from implantable, medical grade Ti-6Al-4V ELI titanium alloy per ASTM F136. The additional implant offering being proposed has similar technological characteristics and identical indications as the currently cleared product line. ### PERFORMANCE DATA Not Required. Cross-Sectional Area calculations, FEA analysis, and projected area calculations show that a new worst-case device was not created. This Finite Element analysis tool was validated by the predicate system's mechanical test data and is sufficient to allow for a substantial equivalence designation for the subject device. #### CONCLUSION The overall technology characteristics and mechanical performance data lead to the conclusion that Genesys Spine Apache® Interbody Fusion System is substantially equivalent to the Genesys Spine Interbody Fusion System (K161438), Genesys Spine Interbody Fusion System (K103034 / K130913 / K153123), and Amendia Turbo Prime IBD System (K080058).
