## Intended Use

The Visibly Digital Acuity Product (VDAP) is a web-based, self-guided software application intended for use by adults, ages 22 to 40, who have the capability to perform a self-test at home, to aid in the evaluation of visual acuity with or without correction. The Visibly Digital Acuity Product does not provide screening or diagnosis of eye health or other disease, nor is it intended to replace an in-person eye exam.

## Clinical Evidence

Prospective, multi-center clinical study (n=329) compared VDAP to ETDRS VA Lane Test. Pooled cohort results: PPV 95.8% (95% CI: 93.6-97.6%), NPV 49.1% (95% CI: 40.0-58.1%), sensitivity 85.5% (95% CI: 81.9-88.9%), specificity 78.8% (95% CI: 69.1-87.6%). Subject-level agreement 75.9%. Reproducibility for study eyes 89.3% (p < 0.0001 vs 75% goal). Bench testing confirmed luminance (ANSI Z80.21-2010) and optotype sizing accuracy (±5% of LogMar).

We have reviewed your Section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (Act) that do not require approval of a premarket approval application (PMA). You may, therefore, market the device, subject to the general controls provisions of the Act. Although this letter refers to your product as a device, please be aware that some cleared products may instead be combination products. The 510(k) Premarket Notification Database located at https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfpmn/pmn.cfm identifies combination product submissions.
