## Intended Use

The IMMAGE® Immunochemistry System Low Concentration Immunoglobulin A (IGMAC) Reagent, when used in conjunction with Beckman Coulter's IMMAGE® Immunochemistry Systems and Cerebrospinal Fluid Protein Calibrator, is intended for the quantitative determination of human immunoglobulin A in serum and cerebrospinal fluid by rate nephelometry.

## Device Story

Device consists of IGALC reagent and CSF protein calibrator for use on IMMAGE Immunochemistry Systems. System utilizes rate nephelometry to measure light scatter from antigen-antibody complexes formed between goat anti-human IgA and IgA in patient serum or CSF samples. Operated by laboratory personnel in clinical settings. Output is quantitative IgA concentration (mg/L). Clinicians use results to assess blood-CSF barrier permeability and intrathecal immunoglobulin synthesis, aiding diagnosis of CNS conditions like meningitis, encephalitis, and tumors. System automates sample processing and measurement, providing objective data for clinical decision-making.

## Clinical Evidence

Bench testing only. Method comparison studies (N=50 per sample type) showed high correlation with predicate devices (r=0.984 for serum, r=0.991 for CSF). Imprecision studies (N=80 per level) demonstrated total CVs ranging from 3.1% to 4.5% across serum and CSF samples, supporting analytical performance claims.

## Technological Characteristics

Rate nephelometry system; goat anti-human IgA antibody; liquid stable reagent; human urine-based calibrator with azide preservative; reaction temperature controlled at 37°C; automated clinical chemistry analyzer platform.

## Regulatory Identification

An immunoglobulins A, G, M, D, and E immunological test system is a device that consists of the reagents used to measure by immunochemical techniques the immunoglobulins A, G, M, D, an E (serum antibodies) in serum. Measurement of these immunoglobulins aids in the diagnosis of abnormal protein metabolism and the body's lack of ability to resist infectious agents.

Dear Mr. Ross:

We have reviewed your Section 510(k) notification of intent to market the device referenced above and we have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (Act). You may, therefore, market the device, subject to the general controls provisions of the Act. The general controls provisions of the Act include requirements for annual registration, listing of devices, good manufacturing practice, labeling, and prohibitions against misbranding and adulteration.

If your device is classified (see above) into either class II (Special Controls) or class III (Premarket Approval), it may be subject to such additional controls. Existing major regulations affecting your device can be found in the Code of Federal Regulations, Title 21, Parts 800 to 895. A substantially equivalent determination assumes compliance with the Current Good Manufacturing Practice requirements, as set forth in the Quality System Regulation (QS) for Medical Devices: General regulation (21 CFR Part 820) and that, through periodic QS inspections, the Food and Drug Administration (FDA) will verify such assumptions. Failure to comply with the GMP regulation may result in regulatory action. In addition, FDA may publish further announcements concerning your device in the Federal Register. Device Name:

IMMAGE® Immunochemistry System Low Concentration Immunoglobulin A (IGALC) Reagent Beckman Coulter™ Cerebrospinal Fluid Protein Calibrator (CSF CAL)

Indications for Use:

The IMMAGE® Immunochemistry System Low Concentration Immunoglobulin A (IGALC) Reagent, when used in conjunction with Beckman Coulter's IMMAGE® Immunochemistry Systems and Cerebrospinal Fluid Protein Calibrator, is intended for the quantitative determination of human immunoglobulin A in serum and cerebrospinal fluid by rate nephelometry.

Clinical Significance:

The concentration ratio of immunoglobulins in CSF and serum detects increased permeability of the blood-CSF barrier and intrathecal synthesis of immunoglobulins.

The permeability of the blood-CSF barrier to plasma increases due to brain tumor, intracerebral hemormage, meningitis, encephalitis, and bacterial infections. The intrathecal synthesis of immunoglobulins is important in the diagnosis of diseases of the Central Nervous Systems (CNS).
