Indications for Use

Device Story

LupoTek Detectin VL and Correctin VL are in vitro diagnostic reagents for lupus anticoagulant (LA) detection in citrated human plasma. Unlike traditional Russell's viper venom, these use Vipera lebetina venom to activate Factor X, which, with Factors V, II, calcium, and phospholipid, generates thrombin to clot fibrinogen. Detectin VL (low phospholipid) screens for prolonged clotting times; Correctin VL (high phospholipid) neutralizes LA to normalize clotting times, confirming presence. PlasmaCon LA is a lyophilized, LA-positive abnormal control plasma used to monitor assay performance. Used in clinical laboratories by technicians/pathologists; results aid clinicians in diagnosing LA-related conditions. Benefits include standardized, reliable LA screening and quality control.

Clinical Evidence

Bench testing only. Precision studies (CLSI EP5-A2) on three lots showed within-device CV < 5%. Method comparison study on 155 patient samples across three sites compared to predicate reagents yielded 98% PPA (91.9-99.4% CI) and 96% NPA (88.0-98.5% CI). Interference testing established limits for hemolysis, icterus, lipemia, and heparin. No clinical prospective/retrospective studies were required.

Test Principle: Vipera lebetina venom, like Vipera russelli (Russell's viper) venom, directly activates Factor X without requiring Factor VII. Factor Xa in the presence of calcium, phospholipid, and Factor V/Va will activate prothrombin to thrombin. Traceability, Stability, Expected values (controls, calibrators, or methods): The reconstituted stability of the LupoTek DetecTin VL, CorrecTin VL and PlasmaCon LA were assessed by longitudinal studies. With criteria of an age-related trend and a maximum shift of 10% of the zero point clotting time, the predicted reconstituted stability of the LupoTek DetecTin VL and CorrecTin VL reagents is 24 hours when stored at 2-8°C and 8 hours when stored at room temperature (23-25°C). The reconstituted stability of PlasmaCon LA is 8 hours when stored at 2-8°C and 4 hours at room temperature. ## d. Detection limit: Not applicable ## e. Analytical specificity: Interference studies of LupoTek DetecTin VL and CorrecTin VL were determined on a Stago Compact analyzer. Interferent was spiked into pooled normal plasma and serial dilutions were prepared. The maximum concentration tolerated in the assay was defined as the highest concentration of interferent relative to any change in the recovered value of the based PNP clotting time was less than 10%. Matrix comparison: Fresh/frozen study: Patient samples were obtained from known normals (n=20) and known LA positive (n=11) donors. Each sample was tested before freezing at -70°C, and after freezing and storage, the samples were thawed in a 37°C water bath. The difference between the fresh and frozen samples was calculated and expressed in raw seconds and in percent of the fresh value. Samples were double centrifuged and testing was performed in duplicate. The geometric mean and standard deviation of the clotting times were calculated and the ranges were calculated as the mean ±2 SD. The mean PlasmaCon LA clot times (as determined in the precision study) with DetecTin VL and CorrecTin VL were 83.7 and 39.7, respectively. Results demonstrate an abnormally prolonged clotting time with DetecTin VL and a corrected time within the normal range with CorrecTin VL. N. Proposed Labeling: The labeling is sufficient and it satisfies the requirements of 21 CFR Part 809.10. O.
