Indications for Use

For the quantitative determination of cross-linked fibrin degradation products (D-dimers) in human plasma on Siemens Healthcare Diagnostics and Sysmex® Coagulation Systems. The INNOVANCE® D-Dimer assay is intended for use in conjunction with a non-high clinical pretest probability (PTP) assessment model to exclude pulmonary embolism (PE) disease and as an aid in the diagnosis of venous thromboembolism (VTE) [deep vein thrombosis (DVT) or pulmonary embolism (PE)].

Device Story

In vitro diagnostic assay for quantitative measurement of D-dimer in human plasma; utilizes polystyrene particles coated with monoclonal antibody (8D3). Sample input triggers aggregation of particles via D-dimer crosslinkage; aggregation detected turbidimetrically as increase in turbidity. Performed on Siemens Healthcare Diagnostics and Sysmex® Coagulation Systems (e.g., BCS®/BCS® XP). Used in clinical laboratory settings by trained personnel. Output provides D-dimer concentration (mg/L FEU); clinicians use results alongside clinical pretest probability (PTP) assessment to rule out pulmonary embolism or aid in VTE diagnosis. Benefits include rapid exclusion of PE in low/moderate risk patients, reducing need for unnecessary imaging.

Clinical Evidence

Multicenter study of 647 patients presenting to the emergency department with suspected PE. Evaluated using Wells' rules for PTP. INNOVANCE® D-Dimer cutoff 0.5 mg/L (FEU). Sensitivity 98.9% (93.9-100.0), specificity 39.6% (35.5-43.8), and NPV 99.6% (97.5-100.0) for all patients. For low/moderate PTP subgroup (n=616), sensitivity 98.6% (92.5-100.0), specificity 40.4% (36.3-44.7), and NPV 99.6% (97.5-100.0).

Technological Characteristics

Quantitative turbidimetric immunoassay. Reagents include lyophilized mouse monoclonal antibodies (8D3) on polystyrene particles, buffer, supplement with heterophilic blocking reagent, and diluent. Energy source is the optical system of the coagulation analyzer (BCS®/BCS® XP). Standard: CLSI EP9-A2 for method comparison.

Indications for Use

Indicated for patients with suspected venous thromboembolism (VTE), including deep vein thrombosis (DVT) or pulmonary embolism (PE), to aid in diagnosis or to exclude PE when used with a non-high clinical pretest probability assessment.

Clinical Sensitivity and Specificity: The INNOVANCE® D-Dimer assay was evaluated on the BCS® / BCS® XP System in a multi-center study to validate the exclusion of PE using fresh specimens collected from 701 consecutive patients presenting to the emergency department with suspected PE. Of these 701 patients, 54 were excluded for a total of 647 patients available for final analysis. All patients were evaluated using the Wells' rules to estimate a high, moderate or low pre-test probability (PTP) of PE. Patient specimens were tested with the INNOVANCE® D-Dimer assay and results were compared to a cutoff value of 0.5 mg/L (FEU) that had been established previously (k081732). A D-dimer result < 0.5 mg/L (FEU) was considered negative and a D-dimer result ≥ 0.5 mg/L (FEU) was considered positive. Patients with a positive D-dimer result and/or a high PTP were evaluated by imaging methods, e.g. spiral CT and/or VQ scan. Patients with a negative D-dimer result and a low or moderate PTP (these patients underwent imaging at the physician's discretion), and patients with negative imaging results, were followed for three months to evaluate potential development of PE. The overall prevalence of PE in those patients available for final analysis was 13.8% (89/647). The following instrument-specific sensitivity, specificity and negative predictive value (NPV) with upper and lower 95% confidence limits (CL) were obtained with the INNOVANCE® D-Dimer clinical cutoff of 0.5 mg/L (FEU).
