The implants are intended to be used with OKTAGON® Bone Level abutments and prosthetic parts. Dental Implant Abutments Bone Level are intended to provide support for prosthetic reconstructions. Prosthetic applications can include individual crowns, bridges, partial or total prostheses. Abutments can be used in single tooth replacements and multiple tooth restorations. The Abutments are intended to be compatible to OKTAGON® Bone Level implants with diameters 3.3mm, 4.1mm and 4.8mm and with the lengths 8mm, 10mm, 12mm and 14mm. The Oktagon Bone Level System is intended for delayed loading, or for immediate loading when good primary stability is achieved and with appropriate occlusal load. ## Device Story Root-form dental implant system; surgically placed in maxillary/mandibular arches; provides support for prosthetic restorations (crowns, bridges, overdentures). Composed of Titanium Grade 4; micro-structured surface (grit-blasted/acid-etched); internal cone connection with octagonal anti-rotation device. Includes locking screw for internal thread occlusion. Used by dental clinicians in clinical settings. Supports delayed or immediate loading (if primary stability achieved). Benefits patients by restoring dental function and aesthetics. ## Clinical Evidence Bench testing only. Fatigue testing conducted per ISO 14801. No clinical data presented. ## Technological Characteristics Root-form endosseous dental implants and abutments. Materials: Titanium Grade 4 (ASTM F67), Titanium Alloy, POM. Surface: grit-blasted and acid-etched. Connection: internal cone with octagonal anti-rotation. Standards: ISO 14801, ISO 7405, ISO 10993-1, ISO 5832-2/3, ASTM F67/F136/F88/F1929, ISO 11137-1/2, ISO 14971, ISO 10993-5, ANSI/AAMI ST79, ISO 11607-1, ISO 17665-1/2. ## Regulatory Identification An endosseous dental implant is a prescription device made of a material such as titanium or titanium alloy that is intended to be surgically placed in the bone of the upper or lower jaw arches to provide support for prosthetic devices, such as artificial teeth, in order to restore a patient's chewing function. ## Special Controls *Classification.* (1) Class II (special controls). The device is classified as class II if it is a root-form endosseous dental implant. The root-form endosseous dental implant is characterized by four geometrically distinct types: Basket, screw, solid cylinder, and hollow cylinder. The guidance document entitled “Class II Special Controls Guidance Document: Root-Form Endosseous Dental Implants and Endosseous Dental Implant Abutments” will serve as the special control. (See § 872.1(e) for the availability of this guidance document.)(2) *Classification.* Class II (special controls). The device is classified as class II if it is a blade-form endosseous dental implant. The special controls for this device are:(i) The design characteristics of the device must ensure that the geometry and material composition are consistent with the intended use; (ii) Mechanical performance (fatigue) testing under simulated physiological conditions to demonstrate maximum load (endurance limit) when the device is subjected to compressive and shear loads; (iii) Corrosion testing under simulated physiological conditions to demonstrate corrosion potential of each metal or alloy, couple potential for an assembled dissimilar metal implant system, and corrosion rate for an assembled dissimilar metal implant system; (iv) The device must be demonstrated to be biocompatible; (v) Sterility testing must demonstrate the sterility of the device; (vi) Performance testing to evaluate the compatibility of the device in a magnetic resonance (MR) environment; (vii) Labeling must include a clear description of the technological features, how the device should be used in patients, detailed surgical protocol and restoration procedures, relevant precautions and warnings based on the clinical use of the device, and qualifications and training requirements for device users including technicians and clinicians; (viii) Patient labeling must contain a description of how the device works, how the device is placed, how the patient needs to care for the implant, possible adverse events and how to report any complications; and (ix) Documented clinical experience must demonstrate safe and effective use and capture any adverse events observed during clinical use. ## Predicate Devices - Dental Implant OKTAGON ([K122807](/device/K122807.md)) - P.004 Implant ([K062129](/device/K062129.md)) - Dental Abutment OKTAGON ([K132214](/device/K132214.md)) - P.004 RC Cementable Abutments ([K072071](/device/K072071.md)) - P.004 NC Cementable Abutments ([K080286](/device/K080286.md)) ## Related Devices - [K162073](/device/K162073.md) — Dental Implants OKTAGON Tissue Level and Bone Level · Hager& Meisinger GmbH · Feb 24, 2017 - [K070007](/device/K070007.md) — SPI ELEMENT PLATFORM O 4.0 MM · Thommen Medical AG · Jan 10, 2007 - [K193401](/device/K193401.md) — Altatec GmbH CAMLOG/CONELOG PROGRESSIVE-LINE Implants · Altatec GmbH · Sep 29, 2020 - [K120503](/device/K120503.md) — CMI IMPLANT IS II ACTIVE · Neobiotech Co., Ltd. · Jun 18, 2012 - [K101225](/device/K101225.md) — PROMIMIC DENTAL IMPLANT, 3.75 MM, LENGTH 8.5 MM MODEL 03-1001, PROMIMIC DENTAL IMPLANT, 3.75 MM, LENGTH 10 MM MODEL 03-1 · Promimic AB · Jan 25, 2011 ## Submission Summary (Full Text) {0}------------------------------------------------ Image /page/0/Picture/1 description: The image shows the logo for the U.S. Department of Health & Human Services.
