Indications for Use

The ABL835 FLEX analyzer is intended for in vitro testing of pleural fluid samples for the pH parameter. The pH measurement of pleural fluid can be a clinically useful tool in the management of patients with parapneumonic effusions. Critical values: pH >7.3 is measured in uncomplicated parapneumonic effusions. All pleural effusions with a pH of <7.3 are referred as complicated parapneumonic effusions; they are exudative in nature.

Device Story

The ABL835 FLEX analyzer is an automated, random-access, stat instrument used in clinical settings for blood gas, electrolyte, metabolite, and co-oximetry analysis. This submission adds the capability to measure pH in pleural fluid. The device utilizes traditional, discrete potentiometric sensors; a pH-sensitive glass electrode (E777) measures potential differences, which are converted to pH values via the Nernst equation. Samples are introduced via a closed fluid transport system, either manually or automatically via the FLEXQ system using bar-coded patient samples. The analyzer provides digital pH output on a touch-screen interface for clinicians. By distinguishing between uncomplicated (pH >7.3) and complicated (pH <7.3) parapneumonic effusions, the device aids in clinical decision-making regarding pleural fluid treatment. Clinical Evidence

Bench testing only. Precision study (n=244-248 per level) showed repeatability (S0) of 0.005-0.013 and total precision (Sx) of 0.019-0.029 pH units. Method comparison study (n=58 samples) against the Roche Omni C predicate showed a slope of 1.063 and an R-value of 0.994. The 95% confidence interval of the linear regression included the identity line (X=Y) across the 7.0-7.5 pH range. Bias at the critical decision point (pH 7.3) was 0.0138, considered clinically acceptable.

Method comparison with predicate device: A correlation study was performed between the proposed device (ABL835 analyzer) and the predicate device (Roche Omni C which is currently marketed as Roche Cobas b221) according to CLSI protocol EP9-A2. This study was performed using 58 pleural fluid samples, pH ranged from 6.93 to 7.57, obtained from 29 patients. Some of the samples were spiked with either NaOH or acetic acid to adjust the pH, so that the samples were evenly distributed in the pH range 7.0 to 7.5. All pleural fluid samples were frozen and thawed prior to testing. Eight samples were tested per day on two analyzers per day over a span of eight days. A single measurement obtained on an ABL835 analyzer was compared to an average of duplicate measurements obtained on the predicate device. Linear regression analysis of the results yielded the following: $$y = 1.063x - 0.446, r^2 = 0.994$$ A bias plot of individual data points showed that all values were within $\pm 0.1$ pH unit of the predicate device.
