## Device Story

System comprises titanium/cobalt-chrome spinal implants (pedicle screws, rods, connectors) and navigation instruments; used in thoracic, lumbar, and sacral spine surgery. Implants feature NanoACTIV micro/nano-roughened surface (1-100 nm) to promote osteogenic differentiation of mesenchymal stem cells. Navigation instruments integrate with Medtronic StealthStation S8 for image-guided placement (CT/MRI/fluoroscopy). Surgeon operates system in OR to stabilize spine or sacroiliac joint; implants facilitate biological fusion. Fenestrated screws allow delivery of saline or radiopaque dye. System provides structural support during healing; NanoACTIV surface enhances bone-implant fixation.

## Clinical Evidence

No clinical data. Evidence consists of in vitro evaluations and imaging studies demonstrating that the NanoACTIV surface (1-100 nm) supports mesenchymal stem cell differentiation and mineralized matrix production compared to non-treated surfaces. Mechanical testing per ASTM standards (F1717, F1798, F2193, F543) confirms structural equivalence.

## Technological Characteristics

Materials: Ti-6Al-4V ELI (ASTM F136), Co-28Cr-6Mo (ASTM F1537), Co-35Ni-20Cr-10Mo (ASTM F562). Surface: NanoACTIV micro/nano-roughened (1-100 nm). Principle: Polyaxial pedicle screw/rod construct; image-guided navigation. Sterilization: Gamma irradiation. Connectivity: Compatible with Medtronic StealthStation S8.

## Regulatory Identification

(1) Rigid pedicle screw systems are comprised of multiple components, made from a variety of materials that allow the surgeon to build an implant system to fit the patient's anatomical and physiological requirements. Such a spinal implant assembly consists of a combination of screws, longitudinal members (e.g., plates, rods including dual diameter rods, plate/rod combinations), transverse or cross connectors, and interconnection mechanisms (e.g., rod-to-rod connectors, offset connectors).(2) Semi-rigid systems are defined as systems that contain one or more of the following features (including but not limited to): Non-uniform longitudinal elements, or features that allow more motion or flexibility compared to rigid systems.

## Special Controls

*Classification.* (1) Class II (special controls), when intended to provide immobilization and stabilization of spinal segments in skeletally mature patients as an adjunct to fusion in the treatment of the following acute and chronic instabilities or deformities of the thoracic, lumbar, and sacral spine: severe spondylolisthesis (grades 3 and 4) of the L5-S1 vertebra; degenerative spondylolisthesis with objective evidence of neurologic impairment; fracture; dislocation; scoliosis; kyphosis; spinal tumor; and failed previous fusion (pseudarthrosis). These pedicle screw spinal systems must comply with the following special controls:(i) Compliance with material standards;
(ii) Compliance with mechanical testing standards;
(iii) Compliance with biocompatibility standards; and
(iv) Labeling that contains these two statements in addition to other appropriate labeling information:
 “Warning: The safety and effectiveness of pedicle screw spinal systems have been established only for spinal conditions with significant mechanical instability or deformity requiring fusion with instrumentation. These conditions are significant mechanical instability or deformity of the thoracic, lumbar, and sacral spine secondary to severe spondylolisthesis (grades 3 and 4) of the L5-S1 vertebra, degenerative spondylolisthesis with objective evidence of neurologic impairment, fracture, dislocation, scoliosis, kyphosis, spinal tumor, and failed previous fusion (pseudarthrosis). The safety and effectiveness of these devices for any other conditions are unknown.”
“Precaution: The implantation of pedicle screw spinal systems should be performed only by experienced spinal surgeons with specific training in the use of this pedicle screw spinal system because this is a technically demanding procedure presenting a risk of serious injury to the patient.”
(2) Class II (special controls), when a rigid pedicle screw system is intended to provide immobilization and stabilization of spinal segments in the thoracic, lumbar, and sacral spine as an adjunct to fusion in the treatment of degenerative disc disease and spondylolisthesis other than either severe spondylolisthesis (grades 3 and 4) at L5-S1 or degenerative spondylolisthesis with objective evidence of neurologic impairment. These pedicle screw systems must comply with the following special controls:
(i) The design characteristics of the device, including engineering schematics, must ensure that the geometry and material composition are consistent with the intended use.
(ii) Non-clinical performance testing must demonstrate the mechanical function and durability of the implant.
(iii) Device components must be demonstrated to be biocompatible.
(iv) Validation testing must demonstrate the cleanliness and sterility of, or the ability to clean and sterilize, the device components and device-specific instruments.
(v) Labeling must include the following:
(A) A clear description of the technological features of the device including identification of device materials and the principles of device operation;
(B) Intended use and indications for use, including levels of fixation;
(C) Identification of magnetic resonance (MR) compatibility status;
(D) Cleaning and sterilization instructions for devices and instruments that are provided non-sterile to the end user; and
(E) Detailed instructions of each surgical step, including device removal.
(3) Class II (special controls), when a semi-rigid system is intended to provide immobilization and stabilization of spinal segments in the thoracic, lumbar, and sacral spine as an adjunct to fusion for any indication. In addition to complying with the special controls in paragraphs (b)(2)(i) through (v) of this section, these pedicle screw systems must comply with the following special controls:
(i) Demonstration that clinical performance characteristics of the device support the intended use of the product, including assessment of fusion compared to a clinically acceptable fusion rate.
(ii) Semi-rigid systems marketed prior to the effective date of this reclassification must submit an amendment to their previously cleared premarket notification (510(k)) demonstrating compliance with the special controls in paragraphs (b)(2)(i) through (v) and paragraph (b)(3)(i) of this section.

K262193 4 of 6

{9}

Engineering analysis and rationale was provided to demonstrate that the subject RIVA Anchor Sacroiliac Fixation System NanoACTIV Screws are substantially equivalent to the predicates RIVA Posterior Fixation System and SOLACE Sacroiliac Fixation System.

In vitro evaluations, imaging studies, and testing were performed to confirm that the deliberately modified NanoACTIV surface produces micro- and nano-scale surface roughness ranging in size between 1-100 nanometers, which exhibit specific osteogenic differentiation. The in vitro study results demonstrated that the modified NanoACTIV nano surface supports differentiation of mesenchymal stem cells through the osteogenic lineage and production of a mineralized matrix as compared to non-treated micro-rough AM titanium surfaces. These evaluations further demonstrated that the endogenous cellular and biochemical response elicited by the NanoACTIV surface on non-additively manufactured titanium was shown to be equivalent to that of the NanoACTIV surface on additively manufactured implants.

## Summary of Substantial Equivalence:

The Xenix Medical RIVA Posterior Fixation System with NanoACTIV Surface Technology is substantially equivalent to its primary predicate, the Xenix Medical RIVA Posterior Fixation System (K251154), with respect to intended use, indications for use, principle of operation, and design: both devices incorporate the same polyaxial pedicle screw, connecting rod, connector, crossbar, and set-screw construct, the same titanium alloy (Ti-6Al-4V ELI per ASTM F136) and cobalt chrome alloy (per ASTM F1537/F562) implant materials, the same subtractive manufacturing process, and the same implant size ranges (Ø4.5–12.5mm, 25–110mm). The principal technological differences are, first, an additional manufacturing step that imparts the proprietary NanoACTIV micro- and nano-roughened surface to the implant, a surface modification with the same nanoscale characteristics (1–100 nm) previously cleared under K233241 and K222988, and second, a change in implant sterility: the predicate device implants are supplied non-sterile for terminal moist-heat sterilization at the healthcare facility, while the subject device implants are supplied sterile via gamma irradiation, consistent with the implants cleared under K233241 and K222988. In vitro osteogenic differentiation and imaging studies characterizing the NanoACTIV surface, together with mechanical testing per ASTM F1717 and ASTM F1798, demonstrate that these differences do not raise new or different questions of safety or effectiveness.

The RIVA Anchor Sacroiliac Fixation System with NanoACTIV Surface Technology is substantially equivalent to its primary predicate, the SI-BONE iFuse Bedrock Granite Implant System (K220195), and to the reference predicates SOLACE Sacroiliac Fixation

System (K233241) and RIVA Posterior Fixation System (K251154), with respect to intended use, indications for use, and principle of operation. Like the predicate and reference devices, the RIVA Anchor implant is a fully threaded, fenestrated, cannulated titanium alloy (Ti-6Al-4V ELI) screw with a self-tapping tip, intended for guidewire-assisted, image-guided placement across the sacroiliac joint, and it connects to the same RIVA modular head used in the RIVA Posterior construct. The device is supplied sterile via gamma irradiation, consistent with the predicate and reference devices, and incorporates the same NanoACTIV surface process as the SOLACE and neoWAVE/Ti3D devices. Differences in implant diameter and length ranges relative to the predicate and reference devices do not alter the fundamental design or principle of operation. Mechanical testing per ASTM F1717, F1798, F2193, and F543, together with engineering rationale, demonstrate that these differences do not raise new or different questions of safety or effectiveness. Accordingly, the RIVA Posterior Fixation System and RIVA Anchor Sacroiliac Fixation System, each with NanoACTIV Surface Technology, are substantially equivalent to the cited predicate devices.

## Conclusion:

The Xenix Medical RIVA Posterior Fixation System with NanoACTIV Surface Technology and RIVA Anchor Sacroiliac Fixation System with NanoACTIV Surface Technology are substantially equivalent to the cited predicate devices with respect to intended use, indications for use, design, function, materials, and performance. The differences in the technological characteristics between the subject device and the predicate devices do not raise new or different questions of safety and effectiveness.
