Indications for Use

The Shoulder Innovation's InSet Total Shoulder System, when used with the InSet Stem, is intended for use as an orthopedic implant for partial or total shoulder arthroplasty to treat the following: 1. Significant disability in degenerative, rheumatoid, or traumatic disease of the glenohumeral joint. 2. Fractures of the humeral head 3. Fractures of the proximal humerus, where other methods of treatment are deemed inadequate. 4. Avascular necrosis of the humeral head 5. Revision where other devices or treatments have failed. The assembled humeral component may be used alone for hemiarthroplasty or combined with the glenoid component for total shoulder arthroplasty. The InSet Total Shoulder System components are intended for single use only. The glenoid component is intended for cemented fixation only; the humeral stem may be implanted by press-fit or cement fixation. N22 Offset Humeral Head made of Titanium is indicated for patients with a suspected sensitivity to the constituents of CoCr alloy that complies with ASTM F75. The wear properties of Titanium and Titanium alloys are inferior to that of cobalt alloy. A Cobalt Alloy humeral head is the recommended component for anatomic shoulder arthroplasty patients without material sensitivity to the constituents of CoCr alloy.

Device Story

N22 Offset Humeral Head is a modular orthopedic implant component for the InSet Total Shoulder System; used in partial or total shoulder arthroplasty. Device replaces damaged humeral head; provides articulating surface for glenoid component or natural glenoid. Operated by orthopedic surgeons in clinical/OR settings. Input is patient anatomy; output is mechanical joint replacement. Titanium alloy construction provides alternative for patients with CoCr sensitivity. Device utilizes Morse-type taper for modular connection to humeral stem. Benefits include restoration of joint function and compatibility with existing InSet system for patients with metal allergies.

Clinical Evidence

No clinical data. Substantial equivalence demonstrated via non-clinical testing including biocompatibility, cleaning, sterilization, shelf-life, axial displacement (taper integrity), material mechanical properties, and range of motion analysis.

Technological Characteristics

Modular humeral head; material: Titanium alloy (Ti-6Al-4V per ASTM F136) with nitrogen ion implantation surface hardening. Connection: Morse-type taper. Dimensions: multiple diameters/heights. Sterilization: not specified. Connectivity: N/A. Software: N/A.

Indications for Use

Indicated for adults (22+) requiring partial or total shoulder arthroplasty due to degenerative, rheumatoid, or traumatic glenohumeral disease, humeral head fractures, proximal humerus fractures, avascular necrosis, or revision. N22 Titanium version specifically indicated for patients with suspected sensitivity to CoCr alloy constituents.

Regulatory Classification

Identification

A shoulder joint metal/polymer non-constrained cemented prosthesis is a device intended to be implanted to replace a shoulder joint. The device limits minimally (less than normal anatomic constraints) translation in one or more planes. It has no linkage across-the-joint. This generic type of device includes prostheses that have a humeral component made of alloys, such as cobalt-chromium-molybdenum, and a glenoid resurfacing component made of ultra-high molecular weight polyethylene, and is limited to those prostheses intended for use with bone cement (§ 888.3027).

The N22 Offset Humeral Head is manufactured from titanium alloy (Ti-6Al-4V) per F136 featuring nitrogen ion implantation surface hardening treatment. The device is available in multiple diameters and heights and utilizes the same Morse-type taper connection, geometry, articulation profile, and system compatibility as the cleared cobalt chromium (CoCr) humeral head component of the InSet™ Total Shoulder System. The N22 Offset Humeral Head is intended for use in patients with a known or suspected sensitivity to the constituents of cobalt chromium alloy. {5} SHOULDER INNOVATIONS K262173 - Page 2 of 3 ### Indications for Use: The Shoulder Innovation's InSet Total Shoulder System, when used with the InSet Stem, is intended for use as an orthopedic implant for partial or total shoulder arthroplasty to treat the following: 1. Significant disability in degenerative, rheumatoid, or traumatic disease of the glenohumeral joint; 2. Fractures of the humeral head 3. The wear properties of Titanium and Titanium alloys are inferior to that of cobalt alloys. A Cobalt Alloy humeral head is the recommended component for anatomic shoulder arthroplasty patients without material sensitivity to the constituents of CoCr alloy. ### Substantial Equivalence Discussion The N22 Offset Humeral Head is substantially equivalent to the humeral head component of the InSet™ Total Shoulder System (K241817) in that both devices have the same intended use, design, geometry, modular Morse taper connection, articulation function, and compatibility with the InSet™ Total Shoulder System. Both devices are intended for use as humeral head components in shoulder arthroplasty procedures. The primary technological difference is the use of Titanium Alloy (Ti-6Al-4V per ASTM F136) in the subject device compared to cobalt chromium alloy in the predicate device. Similar titanium humeral head technology has been previously cleared for use in shoulder arthroplasty applications. The material characteristics of the subject device were evaluated through appropriate non-clinical testing and biocompatibility assessment. These differences do not alter the intended use or fundamental scientific technology of the device and do not raise new questions about safety or effectiveness. ### Non-Clinical Testing Summary: The N22 Offset Humeral Head was evaluated to demonstrate substantial equivalence to the predicate device. Non-clinical testing included biocompatibility evaluation, cleaning validation, sterilization validation, shelf-life validation, axial displacement (taper integrity) testing, evaluation of material mechanical properties, and range of motion analysis. ### Clinical Testing Summary: Clinical testing was not necessary to demonstrate substantial equivalence of the N22 Offset Humeral Head to the predicate device. {6} SHOULDER INNOVATIONS K262173 - Page 3 of 3 ## Overall Conclusion: The proposed subject device has the same intended use and indications for use as the predicate device. Non-clinical evaluation demonstrated that subject device is substantially equivalent to the legally marketed predicate device. The proposed device has similar technological characteristics to the predicate, and the information provided herein demonstrates that: - any differences do not raise different questions of safety and effectiveness; and - proposed device is as safe and effective as the legally marketed predicate device.
