## Indications for Use

The GMK knee cemented prosthesis is designed for cemented application in total knee arthroplasty if there is evidence of sufficient sound bone to seat and support the components. The use of the cementless GMK femur is limited to cases in which the surgeon considers the bone quality to be sufficient for cementless applications. The GMK 3D Metal Tibial Baseplate is indicated for cementless or cemented application if there is evidence of sufficient sound bone to seat and support the components. This knee replacement system is indicated in the following cases: - Severely painful and/or disabled joint as a result of arthritis, traumatic arthritis, rheumatoid arthritis or polyarthritis - Collagen disorders, and avascular necrosis of the femoral condyle - Post traumatic loss of joint configuration - Primary implantation failure Tibial augments cemented are to be attached to the tibial baseplate with both the fixing cylinders and bone cement. The screw-fixed tibial augments are for screwed fixation to the tibial baseplate. In case a semi-constrained liner is used, an extension stem must be implanted both on the tibial and on the femoral components. In case a GMK Revision or GMK Sphere Revision tibial tray is used, an extension stem must be implanted. It is not possible to implant tibial augments, offsets, and extension stems with the GMK 3D Metal Tibial Baseplate and Full PE tibial components. GMK Sphere and GMK SpheriKA can be implanted in kinematic alignment. In this case, this knee replacement system is indicated for: - Severely painful and/or disabled joint as a result of arthritis, traumatic arthritis, rheumatoid arthritis or polyarthritis - Collagen disorders, and/or avascular necrosis of the femoral condyle - Moderate valgus, varus, or flexion deformities

## Device Story

GMK Extension is a line extension for the Medacta GMK Primary and Sphere total knee arthroplasty systems. Components include GMK Sphere FULL PE FLEX/FLEX E-CROSS Tibias (UHMWPE or Vitamin E highly crosslinked UHMWPE) and a GMK Primary Oblique Extension Stem (Ti6Al4V). Tibial components replace the tibial plateau; extension stem provides additional stability to the tibial component. Devices are sterile, single-use, and implanted by orthopedic surgeons in a clinical/OR setting. The system is used to restore joint function in patients with severe knee pain or deformity. Clinical benefit is achieved through mechanical replacement of the joint surface and stabilization of the tibial component. Output is the physical restoration of the knee joint structure.

## Clinical Evidence

No clinical studies were conducted. Evidence consists of bench testing, including comparative dimensional evaluation, anthropometric design optimization, static endurance testing against antero/posterior loads, and comparative performance analysis regarding wear, stability, and range of motion.

## Technological Characteristics

Materials: UHMWPE (ISO 5834-2 Type 1), E-CROSS (Vitamin E highly crosslinked UHMWPE), Ti6Al4V (ISO 5832-3). Components: Tibial plateau replacements and oblique extension stem. Fixation: Bone cement. Sterilization: Sterile, single-use. Design: Modular connection for extension stem; 15 sizes for tibial components.

## Regulatory Identification

A knee joint patellofemorotibial polymer/metal/polymer semi-constrained cemented prosthesis is a device intended to be implanted to replace a knee joint. The device limits translation and rotation in one or more planes via the geometry of its articulating surfaces. It has no linkage across-the-joint. This generic type of device includes prostheses that have a femoral component made of alloys, such as cobalt-chromium-molybdenum, and a tibial component or components and a retropatellar resurfacing component made of ultra-high molecular weight polyethylene. This generic type of device is limited to those prostheses intended for use with bone cement (§ 888.3027).

Dear Christopher Lussier:

We have reviewed your section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (the Act) that do not require approval of a premarket approval application (PMA). You may, therefore, market the device, subject to the general controls provisions of the Act. Although this letter refers to your product as a device, please be aware that some cleared products may instead be combination products. The 510(k) Premarket Notification Database available at https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfpmn/pmn.cfm identifies combination product submissions. The general controls provisions of the Act include requirements for annual registration, listing of devices, good manufacturing practice, labeling, and prohibitions against misbranding and adulteration. Please note: CDRH does not evaluate information related to contract liability warranties. We remind you, however, that device labeling must be truthful and not misleading.

If your device is classified (see above) into either class II (Special Controls) or class III (PMA), it may be subject to additional controls. GMK Sphere FULL PE FLEX and FLEX E-CROSS Tibias are sterile, single-use, implantable devices used to replace the tibial plateau of the knee joint. They are intended to be an alternative to the metal-backed system (Metal tibial tray + tibial insert) increasing the thickness of the polyethylene compared with an identically sized metal-backed one.

The fixation with Tibial Bone is achieved by bone cement. Two versions are available: one made from UHMWPE (ISO 5834-2 Type 1) and the other from E-CROSS (Vitamin E Highly Crosslinked UHMWPE).

They are available in 15 different sizes, each characterized by a different thickness.

The GMK Primary Oblique Extension Stem is a sterile, single-use, implantable device designed for use in total knee arthroplasty as part of the GMK Primary Total Knee System.

It is available in a single size and is manufactured from Ti6Al4V in accordance with ISO 5832-3.

It is intended to be assembled exclusively with compatible tibial baseplates of the GMK Primary Total Knee System to provide additional stability to the tibial component.

- Cement pockets
- Keel design
- Fixation
- Biocompatibility
- Device usage
- Packaging
- Shelf-life
- Sterilization

# GMK Primary Oblique Extension Stem

The subject GMK Primary Oblique Extension Stem and predicate device (K133630) are substantially equivalent with respect to the following characteristics:
