The Augment Off-Axis Instrument System consists of specialized instruments intended to prepare anatomy prior to implantation of Comprehensive® Reverse Shoulder System components in reverse total shoulder arthroplasty procedures or Alliance® Augmented Glenoid System components in anatomic total shoulder arthroplasty procedures in accordance with their respective cleared indications for use and contraindications. Augment Off-Axis Instrument System comprises specialized reusable surgical instruments; designed for preparation of glenoid bone and tissue anatomy prior to implantation of Comprehensive Reverse Shoulder System or Alliance Augmented Glenoid System components. System provides alternate approach for ensuring proper preparation for glenoid implant augments in reverse and anatomic total shoulder arthroplasty. Instruments housed in drop-in tray for storage, sterilization, and transport. Used in clinical/surgical settings by orthopedic surgeons. Device represents minor design and labeling updates to previously cleared predicate (K241043) following complaint investigations.

A shoulder joint metal/polymer semi-constrained cemented prosthesis is a device intended to be implanted to replace a shoulder joint. The device limits translation and rotation in one or more planes via the geometry of its articulating surfaces. It has no linkage across-the-joint. This generic type of device includes prostheses that have a humeral resurfacing component made of alloys, such as cobalt-chromium-molybdenum, and a glenoid resurfacing component made of ultra-high molecular weight polyethylene, and is limited to those prostheses intended for use with bone cement (§ 888.3027).

We have reviewed your section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (the Act) that do not require approval of a premarket approval application (PMA). You may, therefore, market the device, subject to the general controls provisions of the Act. ## Registration and Listing
Owners or operators of places of business (also called establishments or facilities) that are involved in the production and distribution of medical devices intended for use in the United States (U.S.) are required to register annually with the FDA. This process is known as establishment registration.
Congress has authorized FDA to collect an annual establishment registration fee for device establishment registrations. There are no reductions in annual establishment registration fees for small businesses or any other group.
