## Indications for Use

Phasix™ ST Parastomal Hernia Mesh is indicated for use in the reinforcement of soft tissue, where weakness exists, in procedures for the repair of parastomal hernias.

## Device Story

Phasix™ ST Parastomal Hernia Mesh is a sterile, single-use surgical mesh for parastomal hernia repair. It consists of a bioabsorbable poly-4-hydroxybutyrate (P4HB) mesh co-knitted with polyglycolic acid (PGA) fibers, coated on the visceral side with an absorbable hydrogel (sodium hyaluronate, carboxymethylcellulose, and polyethylene glycol). The device features a thermoformed, anatomically shaped channel designed to accommodate lateralized bowel during Sugarbaker technique repairs, with a two-colored PGA marker to assist in orientation. Used by surgeons in clinical settings, the mesh is implanted to reinforce soft tissue; the hydrogel coating minimizes tissue attachment to underlying organs while the mesh facilitates fibroblastic response and tissue ingrowth. The device is absorbed over 12-18 months. It provides a mechanical scaffold for hernia reinforcement, with the anatomical channel improving usability compared to flat meshes.

## Clinical Evidence

No clinical data provided. Evidence consists of bench testing (physical/functional characteristics, degradation profiles, usability) and a 4-week GLP animal study (swine model) comparing the subject device to the predicate (Biodesign® Hernia Graft) in a simulated Sugarbaker repair, showing comparable biological performance.

## Technological Characteristics

Materials: P4HB monofilament, PGA multifilament, HA/CMC/PEG hydrogel coating. Thermoformed anatomical channel. Sterilization: Ethylene Oxide (SAL 10^-6). Dimensions: 18cm x 18cm. Principle: Mechanical reinforcement of soft tissue via surgical implantation. Absorption: P4HB (12-18 months), PGA (5 months), Hydrogel (<30 days).

## Regulatory Identification

Surgical mesh is a metallic or polymeric screen intended to be implanted to reinforce soft tissue or bone where weakness exists. Examples of surgical mesh are metallic and polymeric mesh for hernia repair, and acetabular and cement restrictor mesh used during orthopedic surgery.

# **Device Description:**

Phasix™ ST Parastomal Hernia Mesh is a sterile, single-use device for prescription use only. It is comprised of bioabsorbable poly-4-hydroxybutyrate (P4HB), co-knitted with polyglycolic acid (PGA) fibers, identical to the reference device (Phasix™ ST Mesh). P4HB degrades through hydrolysis and a hydrolytic enzymatic digestive process and is essentially completely absorbed in 12-18 months. Phasix™ ST Parastomal Hernia Mesh is coated on the PGA surface with an absorbable, chemically modified sodium hyaluronate (HA), carboxymethylcellulose (CMC), and polyethylene glycol (PEG) based hydrogel. The hydrogel is identical to the reference device (Phasix™ ST Mesh). The fascial side of the mesh allows for a prompt fibroblastic response through the interstices of the mesh, allowing for complete tissue ingrowth. The visceral side of the mesh is the absorbable hydrogel coating, which separates the mesh from underlying tissues and organ surfaces to help minimize tissue attachment to the mesh. Once hydrated, the hydrogel coating is essentially absorbed from the implant site in less than 30 days. The subject device has the identical intended use as the predicate and reference devices: soft tissue repair/reinforcement. The subject device involves a design change to the reference device, Phasix™ ST Mesh (K143380), with the modification consisting of the addition of a thermoformed, anatomically shaped channel. This channel is designed to accommodate the lateralized bowel in a Sugarbaker technique for parastomal hernia repair. The channel feature has the same absorbable hydrogel coating as the visceral side of the mesh and has a two-colored PGA marker around the perimeter to aid in determining mesh sidedness and placement over the lateralized bowel. This design modification facilitates the usability in parastomal hernia repair over flat mesh, specifically those repairs using a Sugarbaker technique.

# **Indications for Use of Device:**

Phasix™ ST Parastomal Hernia Mesh is indicated for use in the reinforcement of soft tissue, where weakness exists, in procedures involving the repair of parastomal hernias.

# **Performance Data:**

The following performance data is provided in support of substantial equivalence determination.

# **1. Performance Standards:**

No performance standards have been established for this device under Section 514 of the Federal Food, Drug and Cosmetic Act.

# **2. Biocompatibility Testing:**

All of the device materials are identical to the materials used in the currently marketed Phasix™ ST Mesh (P4HB, PGA, ST hydrogel) and Phasix™ ST Parastomal Hernia Mesh contains less hydrogel and PGA than the largest Phasix ST Mesh (K143380). The thermoforming process used to create the channel is already performed on GalaFLEX 3D Scaffold (K161092) and occurs at a lower processing temperature than other existing manufacturing processes for P4HB and PGA. Thermoforming does not impact biocompatibility. Phasix™ ST Parastomal Hernia Mesh has undergone testing to hydrogel absorption (essentially complete in < 30 days), including cytotoxicity, sensitization, irritation, acute systemic toxicity, material-mediated pyrogenicity, subacute toxicity, implantation, and genotoxicity testing, in addition to chemical characterization.

Chemical characterization will support the subchronic toxicity, chronic toxicity and carcinogenicity endpoints for Phasix™ ST Parastomal Hernia Mesh.

K261543, Page 5 of 9

{9}

As the subject device mesh materials are identical to the materials utilized in the successful clearance of the reference device, Phasix™ ST Mesh (K143380), long-term implantation studies conducted on Phasix™ ST Mesh are representative of the subject device. Refer to Table 1.

The biocompatibility evaluation for Phasix™ ST Parastomal Hernia Mesh was conducted in accordance with the Guidance for Industry and Food and Drug Administration Staff Use of International Standard ISO 10993-1, “Biological evaluation of medical devices – Part 1: Evaluation and testing within a risk management process” September 8, 2023, and International Standard ISO 10993-1 “Biological evaluation of medical devices – Part 1: Evaluation and testing within a risk management process,” as recognized by the FDA.
