The device is not intended as a substitute for a complete examination by the dental professional or their clinical judgment that considers other relevant information from the patient's history or actual in-vivo clinical assessment. Final diagnoses and patient treatment plan are the responsibility of the dental professional.

## Device Story

V4D 3D is a cloud-hosted, web-based software platform for dental professionals; it processes CBCT radiographs to perform automated detection and segmentation of anatomical structures. Input consists of DICOM scans (in .zip format) uploaded via a web interface; the backend API routes these to an AI-Worker engine for ML-based analysis. The system outputs visual detections of anatomical structures and a panoramic viewer with automated dental arch identification; it also supports STL file export for educational/research purposes. The software acts as a concurrent read tool, providing additional information to assist the clinician during radiographic examination. It does not replace clinical judgment or complete examination; final diagnosis and treatment planning remain the responsibility of the dental professional.

## Clinical Evidence

Bench testing only. Performance evaluated using 127 de-identified digital CBCT images from 4 US dental offices. Metrics included DICE coefficient, sensitivity, specificity, and tooth labeling accuracy. Results: Airway (DICE 0.968, Sens 99.2%), Mandibular canal (DICE 0.914, Sens 96.8%), Mandible (DICE 0.952, Sens 100%), Maxilla/cranium (DICE 0.951, Sens 100%), Maxillary sinus (DICE 0.96, Sens 100%), TMJ (DICE 0.950, Sens 96.9%). Teeth assessment showed DICE 0.957, Sensitivity 96.9%, Specificity 99.4%. Overall tooth labeling accuracy was 98.6%.

# **Performance Data**

# **Software Verification and Validation Tests:**

Software verification and validation testing were conducted, and documentation was provided as recommended by FDA's Guidance for Industry and FDA Staff, "Content of Premarket Submission for Device Software Functions." Verification of the software was conducted to ensure that the product works as designed. Validation was conducted to validate the design and the performance of the device to meet user needs and intended uses. VELMENI for DENTISTS (V4D) 3D passed all verification and validation software tests.

# **Bench Testing:**

The standalone analysis evaluating segmentation and correct labeling demonstrated the Velmeni 3D CBCT algorithm performance. 127 de-identified digital images were collected from 4 separate US dental offices representative of the general population. The data collected had representation from 3 manufacturers(INSTRUMENTARIUM DENTAL, Planmeca and Sirona) with 20 or more cases, constituting 87% of the test cases. A subgroup analysis was performed that included the following categories: scanner type, sex, and age categories (≤40, 41-64, >64 and 15-21, 22-49, and >= 50).

Based upon the bench testing performed it was determined that the proposed device performed similarly to the predicate
