## Device Story

Monofix PDO is an absorbable, knotless wound closure device; consists of a synthetic, monofilament polydioxanone (PDO) suture dyed violet. Features uni-directional barbs along the length and a stopper at one end for tissue anchoring; includes a stainless steel surgical needle for insertion. Eliminates need for manual knot tying during soft tissue approximation. Used by clinicians in surgical settings. Device provides mechanical tissue approximation; barbs secure tissue position; material is absorbed by the body over time. Benefits include simplified closure procedure and reduced operative time compared to traditional suturing.

## Clinical Evidence

No clinical data. Substantial equivalence demonstrated via bench testing, including USP <861> (diameter), USP <871> (needle attachment), USP <881> (tensile strength), in-vivo tensile strength, in-vivo barb holding strength, tensile strength retention, and biocompatibility per ISO 10993-1.

## Technological Characteristics

Material: Polydioxanone (PDO) monofilament; dyed violet (D&C Violet No. 2). Features: Uni-directional barbs, stopper, stainless steel needle. Dimensions: USP size 2 to 3-0; length 15-60 cm. Sterilization: Ethylene oxide (ISO 11135-1). Standards: USP 43-NF38, ASTM F1980-21, ISO 10993-1.

## Regulatory Identification

An absorbable polydioxanone surgical suture is an absorbable, flexible, sterile, monofilament thread prepared from polyester polymer poly (p-dioxanone) and is intended for use in soft tissue approximation, including pediatric cardiovascular tissue where growth is expected to occur, and ophthalmic surgery. It may be coated or uncoated, undyed or dyed, and with or without a standard needle attached.

We have reviewed your section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (the Act) that do not require approval of a premarket approval application (PMA). You may, therefore, market the device, subject to the general controls provisions of the Act. Although this letter refers to your product as a device, please be aware that some cleared products may instead be combination products. The 510(k) Premarket Notification Database available at https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfpmn/pmn.cfm identifies combination product submissions. #### DEVICE DESCRIPTION

MONOFIX PDO is an absorbable knotless wound closure device comprised of a poly-dioxanone suture. The Poly-dioxanone is a synthetic, absorbable monofilament material dyed violet (D&C Violet No. 2). It is provided sterile. MONOFIX PDO has a surgical needle at one end for soft tissue insertion and a stopper at the other end for tissue anchoring. The suture itself is a Barb-type, designed with small uni-directional barbs along the length. The barbs and stopper eliminate the need to tie knots during usage. The device is available in various lengths and diameter sizes with various stainless steel surgical needles attached to one end.

### SUMMARY OF NON-CLINICAL TESTING

Testing was performed in accordance FDA's guidance document, "Class II Special Controls Guidance Document: Surgical Sutures – Guidance for Industry and FDA Staff" issued on June 3, 2003 and to the following standards to demonstrate safety based on current industry standards:

- USP 43-NF38 <861> Sutures – Diameter
- USP 43-NF38 <871> Sutures – Needle Attachment
- USP 43-NF38 <881> Sutures – Tensile Strength
- ISO 11135-1 Sterilization of health care products – Ethylene oxide – Part 1: Requirements for development, validation, and routine control of a sterilization process for medical devices
- ASTM F1980-21 Standard Guide for Accelerated Aging of Sterile Barrier Systems for Medical Devices.
- Biocompatibility testing in accordance with FDA's Guidance "Use of International Standard ISO 10993-1, "Biological evaluation of medical devices – Part 1: Evaluation and testing within a risk management process" (Issued September 2020)

Other testing included in-vivo tensile strength, in-vivo barb holding strength and tensile strength retention testing. The results of testing indicate that the MONOFIX PDO is substantially equivalent to the predicate device.

### CONCLUSION

The MONOFIX PDO is considered substantially equivalent to the predicate devices based on the testing performed, the similar indications for use, and similar technological characteristics.
