Intended Use
The non-absorbable suture anchors are intended to reattach soft tissue to bone in orthopedic surgical procedures. The OTS 25-L All-Suture Anchors may be used in either arthroscopic or open surgical procedures. After the suture strands are anchored to the bone, both may be used to reattach soft tissue, such as ligaments, tendons, or joint capsules to the bone. In conjunction with appropriate postoperative immobilization throughout the healing period, the suture anchor system stabilizes the damaged soft tissue.
Device Story
OTS 25-L is an all-suture anchor for soft-tissue fixation; expandable push-in design; provided preloaded on disposable inserter. Device components: braided polyester suture sleeve; stainless-steel inserter, drill bit, and drill guide; polypropylene drill guide obturator. Used in orthopedic surgery (arthroscopic or open) by surgeons to reattach ligaments, tendons, or joint capsules to bone. The anchor is placed into bone; suture strands are used to secure soft tissue. The subject device allows the extension component above the bone surface to be optionally packed with autologous bone marrow to augment soft tissue healing. Device stabilizes damaged tissue during the healing period, supported by postoperative immobilization. Benefits include secure tissue-to-bone fixation.
Clinical Evidence
No clinical testing performed. GLP animal study conducted on 20 animals (8 at 6 weeks, 12 at 16 weeks) comparing subject device (with autologous bone marrow) to predicate (K233429) in a Bankart surgical model. Assessments included blood collection, body weight, lameness, CT imaging, macroscopic analysis, microCT, histopathology, and biomechanical testing. Results demonstrated equivalence in tissue healing and attachment strength.

A smooth or threaded metallic bone fixation fastener is a device intended to be implanted that consists of a stiff wire segment or rod made of alloys, such as cobalt-chromium-molybdenum and stainless steel, and that may be smooth on the outside, fully or partially threaded, straight or U-shaped; and may be either blunt pointed, sharp pointed, or have a formed, slotted head on the end. It may be used for fixation of bone fractures, for bone reconstructions, as a guide pin for insertion of other implants, or it may be implanted through the skin so that a pulling force (traction) may be applied to the skeletal system.

We have reviewed your section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (the Act) that do not require approval of a premarket approval application (PMA). You may, therefore, market the device, subject to the general controls provisions of the Act. Although this letter refers to your product as a device, please be aware that some cleared products may instead be combination products. The 510(k) Premarket Notification Database available at https://www.accessdata.fda.gov/scripts/cdrh/cfdocs/cfpmn/pmn.cfm identifies combination product submissions. The control device consisted of the cleared OTS 25 anchor (K233429) and the investigational device was the subject OTS 25-L anchor packed with autologous bone marrow/bone fragments. Animals underwent a Bankart surgical procedure involving unilateral exposure of the glenohumeral joint, a bone lesion creation, and OTS test articles implantation. In vivo assessments performed included blood collection, body weight, lameness, CT imaging (16 weeks only). Ex-vivo assessments included macroscopic analysis, microCT analysis, histopathology (organs and shoulder samples), and biomechanical testing (6 weeks only).
No clinical testing was performed to establish equivalence.
The results of the test demonstrated the investigational device was at least equivalent to the cleared predicate in terms of tissue healing and strength of attachment. | |
