Intended Use
The Flex-Thread™ Ulna Intramedullary Nail System is intended for use in the fixation of fractures and osteotomies of the ulna.
Device Story
The Flex-Thread™ Ulna Intramedullary Nail System is an orthopedic fixation device designed for the ulna. It consists of an intramedullary nail featuring a flexible threaded tip and cortical screws. The device is inserted into the intramedullary canal of the ulnar diaphysis via an antegrade approach. The flexible tip engages the canal, while cortical screws provide stability and articular support in the proximal ulna and olecranon. It is used by orthopedic surgeons in a clinical or surgical setting to stabilize fractures and osteotomies, facilitating bone healing through rigid internal fixation. The system is provided as a set of varying diameters and lengths to accommodate different patient anatomies.
Clinical Evidence
Bench testing only. No clinical data was provided. Mechanical performance was evaluated according to ASTM F1264, including static four-point bend, static torsion, bending fatigue, and bending fatigue of interlocking screws. Additional testing included insertion torque and implant tip deflection to confirm safety and efficacy.

We have reviewed your Section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (Act) that do not require approval of a premarket approval application (PMA). You may, therefore, market the device, subject to the general controls provisions of the Act. Although this letter refers to your product as a device, please be aware that some cleared products may instead be combination products. ## Technological Comparison
This Traditional 510(k) is submitted to introduce a 4.5mm, 6.5mm and 7.5mm diameter nail in 120mm, 150mm, 210mm, 210mm, 210mm, 240mm, 270mm and 290mm lengths as well as additional end caps in 10mm and 20mm. The proposed additions to the Flex-Thread™ System have the same technological characteristics as the predicate device Flex-Thread™ System.

#### Non-Clinical and/or Clinical Tests Summary & Conclusions 21 CFR 807.92(b)
The Flex-Thread Ulna implants were subjected to several mechanical tests with the intent of proving substantial equivalence to a predicate device. The following tests were identified according to the consensus standard ASTM F1264 - Standard Specification and Test Methods for Intramedullary Fixation Devices:
