Device Story
The RTC 26-C is a single-patient reuse adapter providing an access port within a ventilator circuit; it facilitates the administration of Combivent® (ipratropium bromide and albuterol) medication from a Respimat® inhaler to mechanically ventilated adult patients. The device is installed in the breathing circuit and remains in place for up to 14 days. It features a molded body and cap; the cap must be closed after medication delivery to maintain circuit integrity. The device is operated by healthcare professionals in critical care or ventilation settings. By providing a secure interface for the Respimat® inhaler, it allows for the delivery of bronchodilator medication directly into the ventilator circuit, potentially benefiting the patient by enabling aerosolized medication therapy without disconnecting the ventilator circuit.
Clinical Evidence
Bench testing only. Performance evaluations included placement and usage trials, ventilator cycling (18 days), adapter resistance, cap pressure/leak tests, temperature extremes, shipping trials, tether strength, PEEP loss, drop tests, and attachment cycle trials (500 cycles). Aerosol performance comparison testing (cascade impactor at 28.3 lpm) measured total delivered dose, respirable particle mass, and particle size fractions for albuterol and ipratropium. Results showed the RTC 26-C provided comparable or improved aerosol delivery characteristics (e.g., higher respirable fraction) relative to the predicate device.
Technological Characteristics
Molded body and cap made of thermoplastic elastomer and polystyrene butadiene. Designed with 22mm inlet/outlet ports for standard breathing circuit connections. Includes a molded arrow for flow direction. Not sterile. Single-patient reuse (up to 14 days). No natural rubber latex used.

We have reviewed your Section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food. Drug, and Cosmetic Act (Act) that do not require approval of a premarket approval application (PMA), You may, therefore, market the device, subject to the general controls provisions of the Act. The general controls provisions of the Act include requirements for annual registration, listing of devices. good manufacturing practice, labeling, and prohibitions against misbranding and adulteration.
