Intended Use
The Hummingbird™ Tympanostomy Tube System is intended to deliver a tympanostomy tube in tympanostomy procedures in which the patient is receiving a tympanostomy tube.
Device Story
Hummingbird™ Tympanostomy Tube System (TTS) is a single-use, sterile manual surgical instrument for myringotomy and ventilation tube placement. Device consists of a handle and removable tip assemblies; each tip contains a positioning rod and a pre-loaded ventilation tube within a sharpened sheath. Operated by a surgeon under direct visualization; user manually advances the sheath to create a myringotomy and position the tube; manual actuator on the handle retracts the sheath to release the tube into the tympanic membrane. Designed for bilateral procedures via interchangeable tip assemblies. Used in clinical settings under moderate sedation with local anesthetic. Simplifies procedure by combining myringotomy, tube placement, and suctioning into one device; reduces need for general anesthesia; benefits patient by minimizing anesthesia-related risks.
Clinical Evidence
Clinical performance assessed in two studies (feasibility and multi-site) involving 69 children (136 ears, mean age 2.4 years) undergoing tympanostomy under moderate sedation. 100% of patients received ventilation tubes as planned. No intra-operative or unanticipated adverse events reported. Conversion rates from moderate sedation to general anesthesia were 30% in the first study and 7% in the second, attributed to increased surgical experience and design improvements.

A tympanostomy tube is a device that is intended to be implanted for ventilation or drainage of the middle ear. The device is inserted through the tympanic membrane to permit a free exchange of air between the outer ear and middle ear. A type of tympanostomy tube known as the malleous clip tube attaches to the malleous to provide middle ear ventilation. The device is made of materials such as polytetrafluoroethylene, polyethylene, silicon elastomer, or porous polyethylene.

We have reviewed your Section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (Act) that do not require approval of a premarket approval application (PMA). | Performance<br>Data | Clinical performance data was submitted to assess the performance of the<br>device when placed under moderate sedation with local anesthetic. In two<br>studies (an initial feasibility study followed by a multi-site study), a total of<br>69 children (136 ears) underwent tympanostomy procedures under<br>moderate sedation using the Preceptis TTS. The mean age of the patients<br>was 2.4 years (range of 8 months to 8.9 years). Results: |
| | 100% of the children received ventilation tubes as planned. There were no intra-operative adverse events, no unanticipated adverse events, and adverse event rates were well within peer-reviewed literature reported rates. Moderate sedation was per definition from the ASA guidelines, Continuum of Depth of Sedation: Definition of General Anesthesia and Levels of Sedation/Analgesia. The moderate sedation regimen was determined by the surgeon and anesthesiologist. In these two studies, the surgeon and anesthesiologist chose to use nitrous oxide (50-70%) in all cases. In the 1st study, there were 12 conversions (30%) from moderate sedation to general anesthesia due to surgical challenges. In the 2nd study, 2 cases (7%) were converted. The reduction in the conversion rate was likely due to increased surgical experience and design improvements to the TTS to improve visibility and repeatability. In most cases during the study, the surgeon chose to use a local anesthetic on the tympanic membrane to reduce discomfort for the patient. Under moderate sedation, local anesthesia of the tympanic membrane should be used to reduce the risk of head movement and increase patient comfort. |
