The SpineFrontier Lumbar Intervertebral Body Fusion Device System is intended to be used with supplemental spinal fixation system(s) cleared for use in the lumbar spine.
Device Story
SpineFrontier Lumbar IBF System consists of intervertebral body fusion cages designed for lumbar spinal fusion. Devices are implanted by surgeons in an OR setting to treat degenerative disc disease. Cages are manufactured from PEEK Optima LT1; feature raised surface contours to resist migration; and are available in various dimensions to match patient anatomy. The system requires autogenous bone graft to promote fusion and must be used in conjunction with cleared supplemental spinal fixation systems. The device provides structural support to the intervertebral space to facilitate fusion; benefits include stabilization of the spinal segment and potential reduction of discogenic back pain.
Clinical Evidence
No clinical data was required or provided for this device. Substantial equivalence was established through bench testing, including static and dynamic axial compression, static torsion, static and dynamic shear compression, subsidence, and static expulsion testing.
Technological Characteristics
Materials: PEEK Optima LT1. Design: Intervertebral body fusion cages with raised superior/inferior surface contours. Testing standards: ASTM F2077-03 (axial compression, torsion, shear), ASTM F2267-04 (subsidence), and ASTM Draft Standard F-04.25.02.02 (expulsion).
Indications for Use

Special Controls
*Classification.* (1) Class II (special controls) for intervertebral body fusion devices that contain bone grafting material. The special control is the FDA guidance document entitled “Class II Special Controls Guidance Document: Intervertebral Body Fusion Device.” See § 888.1(e) for the availability of this guidance document.(2) Class III (premarket approval) for intervertebral body fusion devices that include any therapeutic biologic (e.g., bone morphogenic protein). Intervertebral body fusion devices that contain any therapeutic biologic require premarket approval.

# 4. Device Description
The SpineFrontier Lumbar IBF System is a spinal intervertebral body fusion device system intended for intervertebral body fusion of the spine of skeletally mature patients, using autogenous bone graft to facilitate fusion. The system is comprised of devices made of PEEK Optima® LT1, with varying widths, lengths, and heights to fit the anatomical needs of patients. The devices have raised contours on the superior and inferior surfaces that will resist device movement following implant.

The SpineFrontier Lumbar Intervertebral Body Fusion Device System was evaluated in accordance with FDA Document, Class II Special Controls, Guidance Document: Intervertebral Fusion Device, June 12, 2007, and has been found to meet criteria defined in the guidance document; and has been demonstrated to be substantially equivalent to predicate devices in terms of indications for use, function, materials, and performance (mechanical testing). Clinical data was not required for this device. Mechanical testing includes performance assessments per the following recognized test methods:

We have reviewed your Section 510(k) premarket notification of intent to market the device referenced above and have determined the device is substantially equivalent (for the indications for use stated in the enclosure) to legally marketed predicate devices marketed in interstate commerce prior to May 28, 1976, the enactment date of the Medical Device Amendments, or to devices that have been reclassified in accordance with the provisions of the Federal Food, Drug, and Cosmetic Act (Act) that do not require approval of a premarket approval application (PMA). You may, therefore, market the device, subject to the general controls provisions of the Act.
