Intended Use

The Dimension Vista™ Thyroid Stimulating Hormone Flex® reagent cartridge (TSH) is a device intended to measure thyroid stimulating hormone, also known as thyrotrophin and thyrotrophic hormone, in serum and plasma. Measurements of thyroid stimulating hormone produced by the anterior pituitary are used in the diagnosis of thyroid or pituitary disorders.

Device Story

The Dimension Vista™ TSH assay is a homogeneous, sandwich chemiluminescent immunoassay using Luminescent Oxygen Channeling Immunoassay (LOCI™) technology. The device utilizes two latex bead reagents: Sensibeads (coated with streptavidin and photosensitizer dye) and Chemibeads (coated with anti-TSH monoclonal antibody and chemiluminescent dye), plus a biotinylated anti-TSH monoclonal antibody fragment. Sample incubation forms bead-TSH-biotinylated antibody sandwiches; addition of Sensibeads creates bead-pair immunocomplexes. Illumination at 680 nm triggers singlet oxygen diffusion from Sensibeads to Chemibeads, inducing chemiluminescence measured at 612 nm. The signal intensity is directly proportional to TSH concentration. The device is intended for use on the Dade Behring Dimension Vista™ System in clinical laboratory settings. Healthcare providers use the quantitative TSH results to diagnose thyroid or pituitary dysfunction.

Clinical Evidence

Bench testing only. Precision evaluated over 20 days (within-lab CV 2.30-3.06%). Linearity confirmed from 0.008 to 102.9 μIU/mL (r=0.9999). Functional sensitivity (20% CV) is 0.005 μIU/mL. No significant interference from hemoglobin, bilirubin, lipemia, biotin, or dextran 40. No high-dose hook effect up to 30,000 μIU/mL. Method comparison with predicate (n=189) showed y=0.95x+0.21 (r=0.997). Matrix comparison (n=55) confirmed equivalence between serum and Na-heparin, Li-heparin, and EDTA plasma. Reference range established using 297 healthy donors (0.358-3.74 μIU/mL).

On each measuring day, one aliquot was thawed, the Dimension Vista TSH method was calibrated, and the QC material and serum pool run in 5 replicates. The analyte values obtained on each day were averaged over the replicates. At the end of the study, the mean values for each day were graphed against the day, inspected visually, and analyzed by linear regression. The Dimension Vista™ Thyroid Stimulating Hormone Flex® reagent cartridge was shown to be stable up to 366 days. # d. Detection limit: Functional sensitivity was determined by measuring eight serum pools ranging in concentration from 0.004 to $0.021\mu \mathrm{IU / mL}$ . Each sample was tested twice a week for approximately six weeks.
